I went to complete Business Arrangement and took sister Tanner as an agent. Returning to Haven she is not showing on the list of agents. I was counting on her for being my 4th so I could take Depth Hands, but she is not there. Is this a bug ?

She probably bailed on you, those sneaky smugglers.
It's probably a bug though. Did you recruit her via the Inquisitors special dialog options or did you use Cassandra/Varric? I think the History perk option is a bit buggy, but using Varric or Cass should have worked.

I used "history knowledge". I forgot to save for a while so I'll live with the mistake. I'll get "Deft broken finger, crooked tools" 
next time around.
