Apparently the Devs never imagined that someone would romance Blackwall then not rescue him when he turned out to be a liar. So during Spa Day Vivienne asks how are things between Thom Rainier and I. "I left that lying scum to his fate" wasn't an option sadly. Cassandra "marriage" talk at least didn't mention him.

And here I thought after the dozens upon dozens in the main game, Bioware would have made sure this not happen in the DLCs.

Hmm, they may have not accounted for players to actually doing that. Don't get me wrong, they should have made sure. They may have thought you would have another romance flag with someone else considering you left him or the flag should have not activated as if you broke off the romance. Or, maybe a response of.. "Vivienne, your dementia is showing." so you could play off on her..

More a bug than a plothole, I think.
They generally don't seem to handle leaving Blackwall in prison very well - you get the codex for when you never found out who he was in that case.
