I had Flawless Widescreen working fine with ME3 on both my desktop and laptop. A few weeks back, it stopped functioning on both of them (they use the same nVidia driver, BTW).
Has anyone else had this happen, and does anyone know of a fix, or what else could be interfering with FW?

It s time for me to try to adjust the FoV.
I m not sure Flawless ws has any advantage over directly tailoring the value in coalesced ?

Yep, have a similar (the same?) problem and wanted to start a thread as well.
It worked until last week for me, although I had to manually pick the right plugin every time I started it new (after reboot, for example). Since I rebuild my new PC last weekend and installed it new yesterday it keeps forgetting not only the correct plugin, but also the FOV parameter.
So for me it is after reboot: start FLWS - pick ME Plugin - set the correct FOV parameter.
Or is it something different for you?

Just edit the coalesced and make it permanent.
Try running Flawless Widescreen as Administrator.

I tried manually selecting, still doesn't seem to be picked up when the game starts. There are recent reports that it stopped working on some other games as well.

Well, duh, yeah 
I don't even know what Flawless Widescreen is, but there was a major update for windows at about the time it stopped working for you, and this update may have reset some of your setting to defaults (a known issue fixed in a later update). So getting the most recent updates and checking any settings that are suspect in this matter might do something for you? Chances are you already did all of that, but just in case.

And if that doesn't work, probably just needs a new fan belt.
Update: I tried several things, including fitting a new fan belt, that didn't work. Then I tried re-installing the ME3 plugin from within FW, and the FW hotkeys magically began working again. So, for future reference, I shall try that first when FW stops working after an update to Windows or ME3. And I'd suggest you do too

No. I took it out, and then put it back in again. It's completely different
