Can't avoid a certain character's death... what am I missing?

I tried to find if anyone had this problem, but no luck. For some reason, Miranda dies in ME3 no matter what despite the fact that she is loyal in ME2, I met with her in the Normandy Docking Bay, warned her about Kai Leng in the spectre office, and again in the Presidium commons. I gave her the alliance resources, and have not romanced her at all. I always pick the top/helpful choices and even the paragon option at Sanctuary, and did not go to any side missions after Thessia. This is the second playthrough in a row this has happened, yet any previous ones before that she always lived, even though I did the exact same thing. Is there something I'm missing?

When you say paragon option at Sanctuary, you mean the top left dialogue in blue that you chose? I just want to be sure.  The other question I would ask is, Are you sure you imported a save from ME2 with her loyal and not a save that you believed that she was loyal? Just want to be sure. A couple of times I imported a save from ME2 thinking I had this or that character only to find out I never recruited that character or that character died during the suicide mission

 

Also what you describe only happens in a default ME3 playthrough. No matter what the player does in ME3, Miranda will always die in a default playthrough

It is possible that her loyalty flag got corrupted. I just recently finished up the Rannoch arc in a NG+ and all the sudden the game thought I had rewritten the heretics even though I blew them up and in the initial run through of ME3 the game correctly had the heretics listed as blown up. Somehow in between finishing the first run and importing into the NG+ that flag flipped.

If you're on PC, open the file with the Mass Effect 3 Save Editor and check if she's loyal.

I just remembered on this character I used the Genesis 2 to make all the past companions loyal since I started on ME3 with this one. It supposedly makes all companions loyal but maybe it doesn't affect Miranda?

What platform are you playing in?  In Wii U, Genesis 2 is the only way to set the decisions in ME1 and ME2 and Miranda dies no matter what you do except if you romance her (among with all the other requirements you specified).  Actually, it now makes sense that it's a Genesis 2 bug as I've been wondering about this since I played ME3.

 

Also, refer to this wikia article for the details regarding the decisions.

 

If you're playing it on PC, can you send me your save before Sanctuary?  I'm very curious about this issue.

Interesting that happens on the Wii U... and I never had this problem until I used the Genesis. I'll look into that save editor and see what it says!

The save editor shows that she is loyal, so I guess that may not be the problem. Also sorry didn't answer an earlier question, at sanctuary I did choose the paragon dialogue option (blue one at the top left) when talking to Miranda's father. Sorry I'm kind of a noob, how would I go about showing you the save?

 Don't know if anyone else has that problem. The other suggestion is to buy ME2 for your pc and import a save from that playthrough with Miranda loyal so that she does survive ME3

You can upload the save through something like this: https://transfer.pcloud.com/

Recreate the folder structure for your save (something like... Mass Effect 3\saves\your_shepard\) and put the relevant save files inside.

 

You can send the email to yourself via pcloud and PM me the link to the download. :)

 

I had an entire spreadsheet poking through the different plot flags for Miranda so I'm curious as to what the Genesis 2 file generates. :D

Thanks to PoliteHanar, we've managed to track down what makes this fail.  It seems that the loyalty check is bugged for ME3 and that part of the requirement to make her survive ME3 is by having an import ME2 Shepard.  The plot flag for this is 21554.  In Genesis 2 DLC, this is not set at all.

 

In addition, do everything that the guide mentions.

 

I know this is a bit of necromancy powers, but this seems to be the only real thread that talks about this particular issue.
