From this thread, it seems that there's a bug on the Genesis 2 DLC causing Miranda to perish in Sanctuary. This is in line with what I know in Wii U where Miranda will die even if you complete all the requirements. The only way to save her is by romancing her. So, what I'm hoping is that someone has a save where Miranda is not romanced but has completed all the other plot flags necessary to keep her alive and that the Shepard playthrough is generated through Genesis 2 DLC. This has been bugging me for as long a time as I've been playing ME3 and I still don't know why this is happening.
