I have played all the expansions and the game at least 6 times, I still have hidden icons that I cant access. How do I get these to work?

DAI?? Same here! What's the square on the down left corner? I have absolutely no idea what that is! and the square under Jaws of Hakkon DLC,what's that? Is that The Descent? If it is then I've already finished that DLC,it remains hidden somehow.
