I think the child in the epilogue is supposed to be of an age where gender is indeterminate from the voice.

That would make sense, the child in the ending scene could always be the N7 soldier from the E3 trailer. Though I'm not expecting that to be the case.

Shepard wakes up, he feels terrible. He drags the covers off his aching body. Sits up on the bed, promptly pukes. This is like no hangover before. What did he get up to last night?
He hears the covers rustle behind him. He turns slowly wondering who it is this time...
A nude Femshep yawns and stretches, "well this is weird."
Shepard, aghast, runs out the door, gets on the Ark, and never looks back. The Milky Way isn't big enough for two Shepards.

I don't see how Shep would be signing them off before the Reaper war .. he was spending his time in the Brig basically ... with James watching over him. Nobody gave 2 shits about him and his galaxy saving tour until **** hit the fan once again but by that time the Ark was already on its merry way

Very much doubt it. I liked the ending, I thought it was a really sweet and a lovely way to end. 
But I think that's all it was - just a nice ending to tell the players that that was the end of Shepard sadly.

No. Star Grandpa was just an addon to ME3 to make it look "artsy". Unless there is something like a Dragon Age Keep to load past saves, I have my doubts Shepard will be mentioned at all, alive or dead. Because, y'know, MEA is the best place to start the Mass Effect experience.

It'll probably start with some voiceover by the new Anderson stand-in leader fellow talking about how they "had to flee the milky way yadda yadda into the unknown yadda yadda hope for the future etc." while the camera pans out a window showing the new galaxy ahead and maybe a little backstory on the protagonist.
So probably just a rehash of ME1's opening for the nostalgia factor with a little exploration theme thrown in.

Honestly, it probably will be very similar to ME1 in the same way that The Force Awakens was very similar to A New Hope. I really dont see them deviating too much from their tried and true plot devices.

I'm still hoping that it isn't an Ark that gets us to Andromeda. I want the opening scene to be the space battle from the end of ME3. See the fleets arrive, go into battle with the reapers. The Crucible then reaches the citadel and fires, but you don't see the result. A wormhole opens up swallows a bunch of ships and closes leaving them stranded in another galaxy with no way back.
