Who exactly placed these skulls (lore wise not dev wise)? There is one on a rampart built by the inquisition... How did this get there? I can understand the astrariums, but what about these?

I think what the OP is asking is more about the shards themselves than the viewers. In Emprise du Lion there's a Shard on an Inquisition built "tower" which made no sense to me, either.

I loathe those. During no playthrough so far did I bother to collect all of them, also because some of them are hard to find with the search mechanic.
I believe they're set up by either the Venatori or the Red Templars... It's mentioned in one of the codex entries, I believe.

I think what the OP is asking is more about the shards themselves than the viewers. In Emprise du Lion there's a Shard on an Inquisition built "tower" which made no sense to me, either.

Cole says he would do something and does nothing ... like everyone else. I guess you can't even RP that Tranquil are people in Thedas cuz their murder matters to no one in DAI. Tranquil are even lower than elves. Worst thing would to be an elf Tranquil in Thedas. Even Cole doesn't really care based on his lack of action.

The Venatori are both responsible for creation and placement the skulls of the skulls. Added bonus they apparently did the whole thing with the rebel mages somehow supposedly not noticing they were doing it right in redcliff even using a that locked shack to store the failures. Basically they summon a demon into the tranquil then behead them.

As many have pointed out, most shards are in out-of-the-way, hard-to-get-to locations. Killing "random strangers" out in the wilderness is what Quizzy does all game long and no one says boo. Why would they if the Venatori do it as well?

Quizzy is all stabby like Sir Lancelot at the Red Wedding in Monty Phython and the Holy Grail. No wonder Solas has to save Thedas.

I don't think they had to kill them in the actual locations of the individual shards to get them to work but in the proximity to a shard. So long as the people doing the deed had access to at least one shard, they could kill the tranquil anywhere they liked. Given they stored the spare ones in Redcliffe I wouldn't be surprised if they didn't do the majority of the killings there. Then they handed out the skulls to the various parties going off to search for shards throughout southern Thedas.
The fate of the tranquil is something that someone should have been brought to account for. As it is, the reaction of the companions seems to be rather "Oh dear, how sad, never mind." It is all very well voicing outrage and declaring you will do something about it but it means nothing unless you do. I found Bull's comment the most objectionable though, "it's not as though the tranquil were using them for anything." I hope this poor joke referred to the skull after death and not before.
Everyone seems to view tranquil as non people just because they don't have emotions, plus seeing tranquil around the place makes mages feel uncomfortable. I felt far more uncomfortable seeing the skulls around the place once I knew how they had been created and hated how they sang to you when you went near them. Initially I ignored them but then discovered their side quest was actually recorded in the keep, so thought it must be important in some way. So next run I collected them all and was underwhelmed at what I discovered when I opened the final door. Subsequently went back to ignoring them again on principle. I wonder if that wretched quest will ever be important in the future.
As for the shards themselves, I was never clear exactly where they originated but presume it was some sort of magical explosion in the past that scattered them throughout Thedas. Since they are invisible without the occulara, that would account for how some shards came to be on modern buildings, since they were probably attached to the building materials used, without people being aware of it.
I assume occulara placed on Inquisition buildings/ramparts must have been put there on the instructions of Leliana, after we have discovered they have a use, since there is also an occulara skull placed on the war table.

There's at least one person in the Inquisition showing some sympathy for the Tranquil, Minaeve (researcher). Though, she doesn't have any comments on creation of Oculara.

I also totally agree the attitudes towards the Tranquil seem to equal severe prejudice against 'weaker' non-beings, not as living people. Fade spirits seem to get more respect. As i know people irl who could be considered tranquil, it does bother me that they aren't allowed to just be themselves, even though it wasn't necessarily their choice in this story to be that way. Forced change by social mores outside of their control.

The tranquil must be reawakened by having a demon or spirit touch their mind, and then the tranquil must be killed in that very instant. This must take place in a location where there are hidden shards. Otherwise the skull will be no good. The shack in Redcliffe is likely where the bogus skulls were taken. Or maybe these were a bunch of preliminary ones until Alexius and the Venatori got the hang of it.
I think this means that Alexius, Imshael, and Lucius were all involved, as well as Corypheus, obviously.
Alexius learned of the tranquility reversal from Lucius. Imshael demonstrated it for Alexius, to show him exactly what needed to be done. And Corypheus discovered the shards and how they are revealed. I also believe that Imshael told Corypheus about the shards and the elvhen doors/tombs. Makes sense, seeing as how Imshael is one of the Forbidden Ones who lived during the times of the evanuris.

Well I feel better now that there is some kind of explanation as I never had a clue about this in my two games. All I know about the shards is that the I would have literally kicked a few level developers in the balls with their placement of a some of these things.
