After the first cutscene in the game when your character run form the spiders and you touch the hand of the divine, I didn't get the cutscene with Cassandra, instead of that my character just appear here

Looks like this happen when you try to recreate Cullen in the Character Creation to romance Dorian, Bioware really hates gay Cullen

I think this has happened to others but it has never happened to me so I don't know how you can fix it or why it's happening.

Is an old bug that can happen when you play a mage, I don't know why I get it now, it should be fixed, goodbye Cullenquisitor, you took me 5 minutes in the CC

That happens when you have really really really low FPS(I had it happening to me on my old PC once).

 

Set your settings at lower graphics.

remove the MaxFPS parameter form the command line. That is the number one cause of this happening. Either put it back later or, deal with the cutscene FPS lock.

DA:I isn't greatly optimized on PC. Though as I said only time I had that issue was on my old pc once.

 

Plus idk did you installed some mods(like used something to enhance graphics) so in case you did that could also affect your fps a great deal. Also recording game is also fps consuming. So idk which could be issue in here since I don't even know your specs, however only thing I'm 100% sure is that what happened is due low fps.

This use to happen to me on my old computer.  I think I just tried different characters and different races.

My solo rogue reached that area on his own once. It is part of the Haven map. I found a map hole and went to the destroyed Temple of Sacred Ashes. Then I turned around and went north of Haven. This area is north of Haven and east of the Breach. It is the area they used to show the cutscene of escaping the Fade, but in this glitch it seems that the cutscene didn't trigger.

Yep, this. I used to have the stuck-in-Fade event after CC until I removed the command line. This will also interfere in Trespasser when you transfer to the Crossroads. I'm running ultra for DAI and this is the only serious glitch so far but it's self-inflicted, didn't come with the game.

I don't believe I've ever had the pleasure of having a "charcater". Or seeing one in the fade.
