What if Darth Nihilus didn't really die at The Ravager and he followed the Exile to the Unknown Regions so he can feed on the Old Sith Empire, then the Republic until nothing remains? And the only way to close this "wound in the Force" would be by the Exile's death (he/she indirectly created it at the Battle of Malachor V). The main characters would be both the Exile and Revan and there would be playable flashbacks from the Mandalorian Wars.

Well, not a bad idea but I'd like to get a new sith lord though.

I like the idea of playable flashbacks but otherwise, just no. If/when there's a KOTOR 3 we would be best served with a new protagonist and a new villain. Re-using Revan or the Exile as a protagonist (or worse a villain as you propose) would just anger sooooo many fans. The flame war over it would destroy the internet! There are just too many fixed expectations fans have of those characters that there would be no way to bring them back like that without alienating half the game's intended audience.

My idea is for KOTOR 3 to center on the effort to rebuild the Jedi order. Remember by the end of KOTOR 2 there's practically none of the order left intact. Most of the leadership/council was wiped out, their ranks thinned from massive consecutive wars, many of their strongholds and temples destroyed. I know the ending of KOTOR 2 gave us little text blurbs about this or that person going on to help rebuild the order but beyond that we have no idea how it went from near complete ruin to the galaxy spanning peace-keeping order it is again in SWTOR. Think of the potential. Every force user you try to recruit could be a powerful ally or turn into a dangerous threat based on how you choose to handle the situations where you find them. Not to mention with so few active Jedi you'd need to recruit pilots, administrators, rebuild political connections to fill out the organization since Jedi are too few to handle all of the jobs involved in maintaining such an Order.

Personally I'd like to see something a bit different. Perhaps the force has vanished from the galaxy for several decades, and you begin as a (seemingly) mundane investigator sent on a mission to piece together the mystery of why it disappeared. Maybe those rumors of a lost generation ship have something to do with it?

One thing is certain, there should be playable flashbacks of the Mandalorian Wars (and a decent number of them).

Then maybe it should be set mostly in Coruscant, with a new MC replacing the Exile training new jedi in order to rebuild the Order, with Atton, Brianna, Mical, Bao-Dur, Mira and Visas as the new Jedi Council. In the meantime, the Exile and Revan are in the Unknown Regions trying to figure out how to deal with the "true sith".

Well if you guys are willing to accept the novel star wars the old republic: revan as canon it invalidates OP's concept especially when the final 2 chapters goes "want the conclusion then play ToR?" Packed into an ending worse than ME3

well The Old Republic is not canon anymore as much as Kotor 1 and 2 are NOT canon. So his ideas are just as valid as anyone else

Perhaps KOTOR 3 should be not just a sequel to TSL, but also a direct sequel to the original game and an intersequel to TSL so we can see what happened to Revan during those five years.

What's the point? It won't happen.

That's the whole point, they won't make it, so we can headcanon whatever we want.

Disney's official word is up until it conflicts with what disney makes its canon.

EA has exclusive rights to SW games.

EA owns bioware

ToR is made by bioware and is STILL in the life cycle of releasing expansions

Not unreasonable to beleive it is still canon

it is very unreasonable to believe it is still canon. In short, outside of the movies and clone wars(cgi), anything before Disney's take over is not canon anymore. Including Kotor 1 and 2, The Old Republic.

Again ToR is still in its life cycle of releasing expansions so it is not 100% before disney and again read my post. Up until there is a conflict it's canon.

The only reason it exist because ToR make money for Disney. Also Disney doesn't consider it canon within Their Canon(aka new canon) and it does conflict with canon due to that Jedi Order's original planet is not Typhon.
