A few months ago, are new PC was built for me, and I was able to reinstall ME3: The Collectors Edition "digitally" from my Origin Account (I played on a Laptop before this). All of the DLC I previously purchased were automatically reinstalled back into the game, including From The Ashes and Leviathan (I have yet to purchase Omega and Citadel because I need more money for BioWare Points). In the in-game menu, I can see the accomplishments for From The Ashes, Leviathan, and all the others achievements just fine. However, in the Origin Menu (You click on the "Details" insignia on the game's thumbnail, and then click "View Achievements"), the Achievements are faded, as if I never did them. But I did on my Laptop, and because of that the achievements exist in the in-game menu( even on my new PC). So what's wrong? Why are the Achievements for From The Ashes and Leviathan are visible in the in-game menu and not the Origin Menu for the detail of the game?

 

Also, the achievements for Citadel which shouldn't be available at this time (because I need more points) are visible my name ONLY in the in-game menu (There are now pictures of the Achievements faded in black).

Achievements are recorded and kept in different ways between the Origin software and the games. I think the in-game achievements are tied to your local profile save, although sometimes there'll still be hiccups with these when moving to a new PC.

 

I believe Origin achievements should've been recorded when you did them on your laptop... Maybe you were offline when playing the game the first time? Whatever the case, I think you'll have to earn the achievements again (while being sure to be logged in to Origin) in a future playthrough if you want them to show up
