I just tried it on a default world state and it worked fine, you can side with Orsino and kill the clan. Could there be another tile stopping you from selecting those options?

i think the change must have happened when i switched merrill from alive to dead, it changed the act 3 faction without a warning, then when i went to change it back it told me that it would affect both merrill and the clan. I've changed Merrill back to alive, reloaded the Keep, and still getting the same conflict.
should the 'stay with hawke' outcome affect it?
Merrill can only die if you side with the Templars.
I honestly don't know what it is. Why not switch all Merrill tiles including her eluvian and clan, then switch them back on one by one and see if it happens again?
I killed her and sided with the Templars and that was fine, but I never did her personal quests so the clan is alive in my Keep state. In my other one, I sided with the Templars and she's alive, the eluvian is shattered and her clan are alive as well. They're both working fine.
Just remembered something, and someone can correct me if I'm wrong...
To kill Merrill you need to side with Templars and have a rivalry, but not 100% rivalry. Just a little. For her to smash the eluvian you need 100% rivalry so maybe that was the conflict? If you shatter the eluvian then you're locked into 100% rivalry, but to kill her you need a lot less so it wouldn't work in the keep (and somehow affected the clan?)

If you use "did not stay with Hawke" outcome while siding with mages, it means you didn't complete her Act 3 quest. That means you can't kill her clan and that's why that tile is changing back to "clan was not killed".
