Yesterday i did the Imshael quest and i lost my Inquisitor during the fight with Imshael i was playing with Sera at the time and when i moved over to him again he was somewhere underneath the floor i think i could still control him but i could not get him back to the fight i am on Xbox one

If you move far enough out of the fight area, your entire team, including your Inquisitor, will appear next to you and disengage the fight.  I don't know if this area gets closed off during this fight as thankfully I've never had an issue with a bug like this (PS4)!  You could try it though.

Yeah, that happen sometimes, regardless of platform. You can also get stuck behind a rock wall if you take on the three shades at the end of the path during Wrath of Heaven.
