I think I've seen it maybe 7 or 8 times. I'd expect that all but one of those would be games that involved people from my friends list or BSNers doing speedrunning, but I do remember it happening once in a PuG. It was one of those PuGs where I had clearly landed in a group of people playing together who were all good, and I was their fourth random.
As for what seemed to be in common when it happened, well... basically, play Gold with people who usually play Platinum, and then hide behind them.
My first Gold Solo was 10 WS.
Admittedly I was a cheesy as hell Reegar Kroguard against Geth, but given I'd probably played less than 20 gold matches at that point and was mostly doing it for the experience, I was quite impressed with myself.
AFAIR i made a 10ws medal with pugs three times. Since i am mostly playing with friends we don't care about surviving because we all know we will make it (which usually is the case, well usually) and everybody wants to kill something.
So, if i'm in a pug, i start to care about surviving, when the 3ws medal pops up. "I'm not the one going down first, not this time" or something like this is going through my mind and i start playing less aggessively and even using OpsPacks.
I think it has to be the right mixture of decent players, good objectives and a reasonable map to get a 10ws (that is "luck" in my book). If the team is failing in achieving this before wave three its most likely i'm responsible for that. 
. In fact i've seen more 5ws medals for the waves 5-9 than for 1-5.

I've done three 10 WS Gold games on X360.
Two were with my usual crew (Brannigan, ctc91, and Drake) -- it helps that ctc91 is really damn good at the game, so we could leave him to hold one flank while the rest of us murdered enemies on the other flank.
In neither case did we intentionally try for 10 WS. We just happened to have good combinations of characters -- I believe both were all Biotics teams -- so we killed fast.
We're also all veterans so we have all the usual veteran stuff: maxed manifests, know how to work the wave budgets, know where to shoot for double-hits on bosses, etc. etc. etc.

The third 10 WS (actually my first, chronologically) was technically a PUG: me, Brannigan, Drake, and a random guy. Interestingly it was a mixed team, two tech and two biotics. I do know one of the characters has Inferno Grenades which are totally OP, so that helped.
Also this game was on Reactor (non-hazard) which strikes me as a pretty good map for getting 10 WS. There are lots of places you can stand that controls spawns and forces the enemies to funnel towards you so you can murder them easily. And, on the non-hazard mode, the objectives on Reactor are pretty tolerable -- even the 'inside the Reactor hack' isn't *too* terrible with a good team.

I'd like to do 10 WS on Platinum some day, although I'd have to re-learn the X360 controller first. Too much time on PC lately. 
Once upon a time when we wanted for farm credits we would play Glacier and sit in the corner at the top of the stairs on at the corner where the spawn point is on Giant. We had a Juggernaut and 2 flamers or Biotics.
I can't remember any 10WS team games, so that makes my story pretty short.
I'm always taking heavy casualties.
When doing platinum farming long time ago, 10WS was quite frequent as it usually was camping with a volus and a jug plus two dps clasees on upper floor of Glacier, in a reactor room, on FB White in the basement, or under the tower on dagger. Jade, Goddess, and Vancouver worked very well also but Giant was kind of slow, well over 20 minutes while other setups were normally 16-19 minutes with some occasional rocketing when appropriate. IIRC, we had at least one 10 WS on each of those maps.
But in UUG pugs these days, yes, very rare occurrence.
I've only seen those 10 waves survived medals when soloing. With teams, 5 waves medals are the most I've gotten and very rarely (even with strong teams)
Actually 10 WS stops counting after... 10 Waves. 
You can die during extraction and it won't matter.

As for myself: I have indeed been in some Gold PUGs with 10 WS. Although last time this has happened I found out two of the pugs were rarely posting BSNers. Still, it was a PUG and of course, a great game!
I don't think I've ever had a 10WS pug (and I'm probably responsible for that 
). I do have 10WS gold solos (two, with reegar kroguard/geth).
Easier to do in a solo- just be the KroDept. The times I've had it happen in a PUG are usually just completely random, it wasn't even necessarily when you get a lobby of high CP/N7 players or even BSN'ers.
I can't remember the last time..or if this even has ever happened in a game with randoms.. Then again, I do drink when I play. It almost happened the other night in a random pug game...but a primes turret ended that on wave 9.....
