Happened about 5 times during the 10 waves. I leave you to imagine the consequences. Turns out the Lancer is only deadly on full auto 
Only way to fix it was to switch weapons, fire the alt weapon, and switch back. Heavy melee did not fix it either.
Anyone else ever encounter that?

Fill out a bug report and Biower will get right on it 
I had a similiar problem with a few other weapons. Sometimes they'd stop firing and I had to click again so they'd continue their supposed auto fire... then some charge weapons would fire while I was still holding the mouse button down... at first I thought my mouse was broken but it worked fine everywhere else.
I managed to fix the problem by deleting the weapon files in ...\Mass Effect 3\BIOGame\CookedPCConsole and then repairing the installation (thus downloading all of it again). Repairing the game without deleting the files only worked once, then the problem returned. Disadvantage: All mods need to be re-installed, too.

Thanks, Daemion. Will try that. Just finished a (painfully lengthy) Plat solo with the same kit and setup and thankfully had no issues this time, but I'd rather be safe than sorry.

Had invisible Fury tonight, which made sense since it seemed like whenever a banshee got off a sync kill it leveled up and became invisible too. (Seriously happened in front of my eyes. Bloody terrifying.)

You see, I'd be standing a reasonable distance away from them. There was no way they could reach me from where I was standing.

This was chronic for me when I played on Xbox, but I've never had it happen on PC. Could be keyboard driver weirdness as well.
I think it's keyboard too. I suspected one of my X-Box gamepads I was using on PC was acting up (was getting that so called running bug too, among power buttons not registering sometimes), so I plugged in another one and problemo's solved. Hasta la vista cheapo Gamestop stop gag gamepad. You'll make a great kitty toy.
It's occured to me only once so far, when shooting I got staggered by brute. It passed when the next wave began.

Speaking of staggers, if I get staggered as a Cabal doing a teleport dash, I often get stuck and cannot move or do anything, and have to ragequit the match. This is why I don't offhost Cabal anymore. It's happened a dozen times.
I've had the same problem as OP plenty of times.What I started to notice was that when I cycled from the rocket,or 2nd weapon,I had used a power that partially interrupted the animation.Then my automatic gun was semi auto.Sometimes I could fix it by shooting a rocket,or letting an enemy take me down,then gelling.

Though to be fair, if the bug hits you on Gold or Plat, either of those options will most likely present themselves very fast.

Unless the bug hits you when you are out of rockets, out of gels, and all your teammates are out for the round. Of course against Collectors, so you can't really rely on your powers to take that last possessed praetorian down. That was one long and painful wave 10 clean-up 
By the by, does your lancer actually shoot on full auto while activating powers, when hit with this bug? 'cause that's what is happening for me 
you don't hit anything, since you are mostly aiming at the ceiling during the animation, but it can cause the gun to overheat, to add insult to injury

Yes, I repaired the game and it seems to hold up since then, but if it happens again I'll try your more extensive method. I haven't really played the Cabal/Lancer since the Plat solo as I overdosed on them a bit. So far so good with any other kits/weapons.
Happened to me once. I couldn't figure out what the problem was all match until I realized that I had cut the nail too short on my trigger finger, making it too painful to depress the fire button. This led me to single shot firing the Lancer, too. I'm positive this is what is happening to you, too. The similarities are too uncanny to be sheer coincidence.
I would suggest purchasing new nail trimmers. Since I got mine, I haven't had the issue at all. It has changed my life.
