Shiala's biggest flaw was misjudgment. She joined Benezia in some bleeding heart attempt to "guide Saren down a gentler path" instead of just killing him, and all became indoctrinated when they boarded the reaper. After that, Shiala's actions were beyond her control. When passing judgment, morally, it is less about if she deserves to die and more about if letting her go is too dangerous since she was indoctrinated. At that point though, Shepard is likely ignorant about how indoctrination really works.
I believe Shiala is the only character in the series to actually permanently break free from indoctrination; thanks to the thorian joining.
