It says it's acquired upon a second completion of the arena at Stone Bear Hold. But when i go in there a second time i beat a wave of avaar and they just keep coming. i've been in there and have already beaten 6 or 7 rounds and it  doesn't give me a way out. Is this supposed to happen? How many rounds are there the second time?

That sounds like a bug  you're supposed to only get 3 waves of Avvar.

1st wave: 4 Avvar Warriors and 2 Avvar Bowmen.

2nd wave: 2 Avvar Warriors, 2 Avvar Bowmen, and 2 Avvar Defenders.

3rd wave: 2 Avvar Defenders, 2 Avvar Champions, and 2 Avvar Bowmen

 

Return to Trainer Arrken to complete the quest. (x)

I don't know exactly how many rounds there are the second time you enter, but it's a lot! Just keep going, it becomes quite entertaining. Enjoy your sword.

You'll encounter about 10 waves if you go in after you finish the mission you have to do.  It's not a glitch.  You get a fancy weapon.  One of the waves is a bunch of nugs.  Another wave is um.. crap what are those purple venom spitting lizards called? Anyhow, you just have to survive the waves and you get a 2H sword

I don't remember it being 10. Six, tops. I wish it was 10! Loved that quest. Wish there was a permanent arena where you can just keep fighting waves of higher and higher level enemies until your entire party is wiped out and yes I know I'm describing Witcher 2 Arena.

Beat it a few hours ago and it's definitely 10.

Where you playing on Nightmare? That might be what the difference is. I wasn't on Nightmare, if I recall. It was a while ago.
