So, as the title states starting my first playthrough, any mods that really make a difference?

Yes. Any of the hair mods, because most of the default ones are pretty awful. If you have Trespasser and intend to play a female character, I also recommend a wedding dress replacer for the formal attire, with a retexture: one or two
Textile Shop does some lovely things with material textures, too.

My recommendation is to go through your first game clean with no mods. The mods I use are all pretty much just change appearance but there are some out there they can change the game mechanics. You should be aware that some mods aren't compatible with the DLC.
When you start modding the first thing you should get as mentioned above is a hair mod. Doesn't matter which one. All are better than the stock hair and a hair retexture. These are the ones I am using out the moment.
All about the face
DAI Hair Recolor Utility
Spirit Blade Recolor
Eyebrows for Women
Custom Body Models
LL Crafting Materials Metals Only
HD Eye Textures
Opulent Tints or Palletes of Thedas or Seasonal Palettes

I agree with this, with one exception. The Quicker Looting mod arguably changes game mechanics, but it's worth it. If you intend on doing any amount of farming for crafting materials, this mod will literally save you hours of real time. Even if you don't farm crafting mats, it saves time looting chests and corpses.
Use the Quicker Looting - LW file. Otherwise you won't even see what you are picking up and miss the joy of getting a rare drop.

To be honest, I only thought about mods when I tried to create an elf, those tattoos are horrible, and there is no option to not get one at all. The only solution is to make it a very light color, but even that shows up in some cutscenes.

Quicker Looting is indeed a must have. Other than that, I'd limit myself to cosmetic mods. There's little reason not to play with improved hair or face textures if you can.

AFAIK you can add pretty much all mods mid-game. The only caveat is that some face textures and hair cause issues with the Black Emporium - however, as they're replacements for default ones, you can take the mod off, use the Mirror to select the texture/hair that's being replaced, then, once you've left the BE, reinstall the mod.

I don't think you should do mods until you've played the game at least once without them. That said, if you're playing along and there is something that really bugs you, then by all means, go see if there is a mod for it.

I agree I did my first 3 playthrough's without mods but I do agree with what people have asid in that the hairstyles aer a bit weak. But in truth that's the only area which I feel I do need to mod. Yes I have the viv mods but that's moer down to the fact I think Viv's outfits aer sexy and wanting to be able to use them on my characters.

Qunari especially got the shaft where hair is concerned. I loaded the game up for the first time with every intention of making a badass qunari gal, like the concept art
... but I couldn't bring myself to play one until hair mods happened. That horrible little bun. =/
Eyebrows are pretty awful too. There are a couple that are fine but you use the same ones over and over if you don't mod lol.
Hair and wearing that ugly uniform as a lady to Halamshiral were my two biggest "Bioware, whyyyyyy" things at launch.

Also if a mod locks a scene or a DLC from loading correctly you can remove the mod(s) that are the culprits(s) load in and then put the mods back on. Once you have loaded into the area most mods work fine.

EDIT: Alot of mods that have issues are ones that deshine you complexion or hair.

I've been looking at couple of different eyebrow sets, especially the 'Eyebrows 4 women' set. It says dark and fair but I wonder if they still change the color accordingly to the hair color? My elf has light grey eyebrows and I wouldn't want them to change to black.

I thought the "dark" version would not if fair hair was selected, which is what I wanted, blonde hair, dark eyebrows, but unless I messed up, the "dark" eyebrows were just slightly darker yellow for the lightest blonde (not white) hair color. Looked like clown make-up, I'm afraid.

I don't think so now they're done working on DAI it's all about ME Andromeda for them now we should be able to play with them at our own leisure with DAI now.

I disliked the removal of several Amulet of Powers in the patched game, which I guess they did in order to remove the Amulet of Power dupe. As I'd never use the dupe and really liked the "mini-game" of finding them, Amulets of Power - Restored is more or less a must for me.
