For the lolz, they should have the risk of permadeath hanging over the players head every moment of the game, like XCOM or Fire Emblem.  One moment you have your dream team, the next your beloved LI is dead and gone for ever.

There shouldn't be any outside of scripted ones so the narrative doesn't become a mess. If you are doing mulitple installments with the same characters, don't make them killable until the final one so the last game doesn't become a rush job because of the requirement to write 4 different versions of each arc to accomodate everyone, or abandon characters' roles in the narrative because "could be dead".

Also, don't give us retarded deaths for pointless "sacrifice feelz" or because of trying to fit them within a completely arbitrary binary morality system. If someone has to die because we betrayed or oppose them, make them stay in character and offer effective resistance instead of grabbing the idiot ball because "muh choicez". I don't want Miss "I'm willing to die for my Fleet" Tali or Admiral Raan pouting, blubbering and jumping off a cliff/shooting themself when I kill their species because "paragon", instead of turning the gun on me rather than themselves. Or emotionless, calculating, mechanically stoic Legion moronically attempting to give me a bond villain monologue with an armed enemy combatant right behind it, then laughably getting punked. Or, it having to autodie like some kind of Robo-Jesus because it has lost the ability to copypaste files for some inane reason. Mordin's sacrifice when curing the genophage was handled okay, but he was calculating and should not have been the type to turn his back on a gun if he couldn't be talked into the Sabotage. Both of Wrex's were okay apart from the ME1 version again necessitating lengthy rewrites of story arcs that waste tons of time. TIM's Paragon bluespeech induced suicide was perhaps the dumbest of them all.

I just found the entire TIM on the citadel scenario completely ridiculous and couldn't take it seriously at all.

Evety game should come with red shirts. Sucide mission in 2 i made sure a coupke died in every playthru,made it ineresting and more real.
