I totally missed the scene when Sera tells me about the nightmare, when and how does that get triggered? I got the achievement for committing to a romantic relationship though. But, the tarot card still hasn't reached the last tier (romance card). Help please!

This should occur after completing both Wicked Eyes and Wicked Hearts and Here Lies the Abyss. In either case, you will need to trigger scene pertaining to the previous mission first (either Sera frustrated with Orleason nobility or Sera frustrated by the Fade). After the scene, go back and talk to Sera and the nightmare scene should trigger, assuming you have already completed her gift quest, love scene, and rooftop scene (it sounds like you have). I believe this must be completed before you finish What Pride Hath Wrought, but I'm not positive.
