What I know: It happens after getting the third addition to your army. You select a location to go and instead of actually moving there, your command is interrupted by a cutscene with the archdemon.
However, I've done a lot of testing and it seems so freaking random. Sometimes it will happen as soon as you click on a location. And in the next time you reload the game, your party moves to that place normally.
This crap has left me stuck in the game for days, because I'd like the reunion with Tamlen to happen before my next stop in Orzammar. I don't want to spend hours doing Anvil of the Void only to find out that I've skipped over that moment. Then it will be too late.
Could someone explain to me how the Tamlen encounter works?

I'm fairly certain it's not skippable. Even if you can't get it to trigger before going to Orzammar, it should occur the first time you return to the surface. I've never had it skip on me. I played on PS3, so I don't know if other systems had different outcomes though.

Wiki says after 5 main missions are complete, I think.. any order. I've had it before Orzammar, but that's when Orz was absolute last thing on my list. Redcliffe, Circle, Dalish, then I think the ambush happened. But I think Redcliffe might have counted as two missions (Sacred Ashes being the second). Perhaps it counted Ostagar or Lothering as a "mission" as well.. so that would add up to 5.
To put it more simply, if you followed the same order as Varric's "Keep" narrative, you'll get the ambush before Orzammar.
