I don't know what's wrong with my Warriors lately but they die like they are level 8 and they should be level 17. I am a Mage at the moment but i am a better tank then they are. Even Blackwall dies a lot of times and he was a couple of playthroughs ago unbeatable. I use the same build that i used in those previous playthroughs but it's like their AI is changed somehow i don't know. Did Bioware do something in one of their latest patches ?

You're not using an item with Berserk, such as the Wintersbreath shield, are you? That will double the damage done to them.

Very likely ^this.

If not, you may be getting more than your fair share of the stand-and-do-nothing bug. Cycle through your party to make sure they are all doing something.

What difficulty? Nightmare? Which Trials, if any?

Also, sanity check the basics. AI settings: stamina reserve 0, Follow Controlled until you are sure the AI is behaving, take potions at 30% health, reserve no more than 3 if you have 12, 2 if you have 8. Tactics: Prefer Walking Fortress on Blackwall, Wrath of Heaven and Spell Purge on Cass, War Cry and Livid on Iron Bull (and anyone else that has them). Give one of them Horn of Valor/Fortifying Blast on Preferred.

Yes that is another thing i have that bug to when they just stand there and do nothing. Then i cycle to that companion i have to jump to reactivate them otherwise they are like statues. But that happens only to the Warriors.

At the moment i am on Normal mode so pretty easy but i get the feeling that i am on Nightmare mode but only from the Warriors point of view.

It's all check Blackwall has his Walking Fortress Cassandra has her Wrath of Heaven and Spell Purge and so on and so fort. I am using the same build that i used a couple of playthroughs ago and back then it was fine ( more then fine ). I did not have to check on them especially Blackwall he was like on Godmode or something but now he dies so easily and back then i was on Normal mode to.

Well that's just it i don't know if their AI is behaving correctly i cannot check on them all the time. And i have no idea how to fix their AI.

I haven't noticed any change in their AIs in later patches, so I don't think it's caused by changed behaviors. I also get the stand and do nothing bug, but that always (?) means nothing happens, so they aren't killed then. I think attacks makes it work again, at least that seems to be the case when it happens to enemies. In short, no idea what might cause it for you.

So it's a bug then ? Yes true they aren't killed when they are in that stage of doing nothing. And if some enemy attacks them then they are moving again. But sometimes they are just to far out of range that i have to reactivate them by controlling them. Very annoying when that happens.

Yes, it's a bug that crept in with one of the later patches. It's really annoying, happens Cassandra quite often. It's actually annoying when it happens enemies as well as it removes a bit of the challenge.

Yes true Cassandra is the one who gets stuck mostly in my game as well. I have not yet seen it when it happens to an enemy btw. But i will watch more closely from now on and see if that happens in my game as well.
