Attack Rating? What does it mean exactly?

I know attack rating for warrior and rogue is the percentage chance to hit an enemy. Few questions:

 

1. Does the percentage chance apply to a ranged attack? Seems like my ranged attack always hits. 

 

2. For a mage, does the percentage chance apply to a magic attack? Again, it seems like the magic attack always hits. So does it apply to the mages staff attack?

Strictly speaking, all attacks always hit. However, attacks that count as "misses" do only glancing damage, which is significantly lowered.

Spells and Talents always hit (barring a solid obstacle in the way or if you aim badly)

"Attack" as a stat is basically how likely your basic attacks will hit (dealing full damage) or graze (dealing a fraction of the damage)

Defense works the same way, in reverse. It determines how accurate enemy attacks are against you.

Think of it like "Accuracy" and "Dodge", and you have the general idea.
