Not the worst one, but it's carved in my head forever, just hated the whole scene in general (cringe worthiness or not).
 
But that Kai Leng impaling Thane scene, it's what follows with Shepard that cringed the heck out of me:
 
* Kai Leng impales Thane *
Shepard: " Thane! "
* Shepard pursues Kai Leng a moment *
* Kai Leng escapes, Shepard can't aim anymore, Thane just catching up on them, collapsing against a wall nearby *
* Shepard turns around, runs to Thane *
Shepard: " How bad is it? "
Thane: " I have time. Catch him. "
Shepard: " Oh, okay, yeah I can do that. "
Commander Bailey: " Shepard, what's going on up there? "
Shepard: " Just having fun with my bud Thane! * taps Thane on a shoulder, Thane reacts painfully * ...and hummm... wait, actually, I think that Thane is going to need some medical attention. "
 
How bad is it. HOW. BAD. IS. IT.
 
It's nothing Shepard, just a Katana going through a terminally-ill Drell.
 
Not much would have been needed to remove the cringy part. Just let Shepard yell "Thane!", continue the pursue scene as is (even though I hated it all, but whatever, this is just for the sake of the discussion), entirely skip the stupid question Shepard had in mind, have him run to Thane and upon getting close enough have him contact Bailey instead to ask for immediate medical assistance. Then, have the camera focusing on Thane while he looks up at Shepard just telling him to "Catch him", and that's about it, no? No it's not. Nothing is ever simple.

Reminds me of talking to NPCS during side quests in DAI.

NPC: I want you to do something.

Inquisitor: you said you wanted me to do something. What do you want me to do?

I don't remember cringing much. DA is capable of doing that to me more.

 

The only thing off the top of my head is when I tried to romance Liara. Those blank eyes freak me out.

I was crazy enough to do those damn collectibles again in my last (and final) playthrough. No stone unturned..... for no good reason whatsoever.

It started with stuff like the remains of Sister Plinth in DA2.

"Oi, those bones look familiar. Lemme get my backpack, I just know where to drop these off..."

Then you click on the NPC (supposedly handing over a sack of decaying human bones) and get the line "Look what I've found... your reason for living."

That in my opinion started the bad, cheap side quests era that continues to this day.

On topic: everything about Kai Leng made me cringe. Each time he showed up it felt like the devs were just about to break the 4th wall. "See, kids, this is how plot armor works. Next stop: Thessia!"

This. Seriously, this. Then you see Palaven on fire behind you ... each time I play, I feel so ashamed, bad, guilty that my Shepard asks that, without any real compassion, like it's not a big deal for them when everybody is dying on their planet. This is not who I want her to be or who I am. It kills my roleplay immersion !

 

I can't stand the "Earth is sooooo special" thing at the beginning of ME3 in general.

 

And everything about Star child's...
