I originally assumed it means the character fundamentally disagreeing with me. But that can't be the case, since I have Varric on red now. And he never once disapproved my decisions.
What does it mean if a companion card turns red?

Did you rescently complete his personal quest with Bianca? His card will go red if you finished that quest, but have not yet had the Wicked Grace game yet.

His card changes to this one for a short while. There's going to be one more change.
And no, tarot cards don't change like that. You have basic cards, romance cards and cards that appear at specific points in the game (mostly after completing companions' personal quests). If companion leaves Inquisition, their card has "x" on it.

Varric's card turns red after Here Lies the Abyss, just do his Well Sh!t companion quest and it will/should change again.

I have Cole on constant code red too. But he actually doesn't like most of what I do.

It's not necessarily that the card "turned red" and red is bad, but it's an entirely new card with different artwork. Some of the followers have reddish cards during certain points in the game, but not all.

You will not get his cut scene when you speak to him in Trespasser without completing his personal quest with Bianca. Otherwise, I don't recall any difference in his demeanor.

It annoyed me that a Bull loyal to the qun doesn't have a Trespasser scene. Yes, I realise he betrays us, but come on. Even if he knew the betrayal was coming, there's no reason not to be civilised about it, have one last goodbye. We were partners (and possibly lovers) for over two years; that deserves more than "nothing personal, bas."
Hell, Solas plans to kill us and everyone else in the world, and we still got a moment with him.

On a side note, where do you guys see how much time has passed? I looked everywhere in the menu, but I can't find it.

There's no way to check time.
Those two years everyone's talking about is time between the end of Inquisition and the start of DLC Trespasser. It's mentioned a few times in DLC itself as well as in a trailer to it.

To add to this, the war table warning for starting the Trespasser DLC with the "Attend the Exalted Council" operation says, "Warning: Beginning the Exalted Council will lock all other areas and plots and jump to 9:44 Dragon, two years after the death of Corypheus. There is no going back from this point."
