Why did ____ die???

1. Jacob died when I sent him through the vent. Should have known he was bad at tech... oops
2. Zaeed died when I made him lead team 2 in the second part... don't have a clue why
3. Moridin was shown randomly dead when leaving...?
I did all the loyalty quests and got all but the extra fuel and plastic surgery upgrades so... I guess don't send Zaeed to lead the group? And take Moridin with you at the end or else he dies? Help!

Zaheed led the alternate party both times, died the 2nd.
And the ships crew was killed on the way back (I chose to not send someone with them.)
I'd like to discover what exactly determines success or failure, I had every upgrade I knew about, explored the whole galaxy and had all side quests completed.

I've only seen Garrus leading the other team and he's always lived through that.

Always use Garrus to lead the secondary Squad, he has the best leadership skills from being in the Military, C-Sec, Working with you in ME1 AND from having his own faction in Omega.
When it comes to the biotic needed to pass through the swarms of bugs, use Jack and have a squad consisting of Samara and Zaeed.
THEN, at the final task, take Garrus and Miranda with you.
Since these are the right people for the billets required, they won't die. The less people that die, the more people you have watching the other squadmates backs, preventing them from dying by default.
This will keep everyone alive, but only if you have the loyalty of everyone, else some people might back out/(dis)obey orders.
Also, send Jacob back with the shipmates.

- sent Legion through ducts, he survived
- Jacob led the second squads every time and survived
- Samara did the swarm shield, team was the Mordin and Zaeed, all survived
- Sent Tali back with the teammates, all survived
- final team was again Mordin (who had randomly died when left back with the rear guard) and Zaeed, and Zaeed died when you fall after the boss dies.
Did anyone manage to keep Zaeed alive without having his loyalty?

Zaeed isn't a good leader. He'll die the second time if you make him squad leader, and your tech will die regardless of their compatence/loyalty if he is made first squad leader.
Mordin is apparently a trouble character for a lot of people. Personally I've never had him randomly die, but a lot of people have. Send him out to escort the crew back to the ship to be safe.

He can escort the crew back to the ship, though. If he's loyal.
Tali or Legion are your best bet for the pipeline mission. If you have a good leader for the second team then they will almost certainly survive.
Miranda, Jacob, and maybe Garrus are good leaders.
Samara and Jack are good biotics.
The two members you bring with you for the final run are rather tricky. I hear that it has something to do with loyalty and maybe even alternate costumes.

Miranda, if loyal, will keep everyone alive and stay alive herself as the secondary squad leader for both sections. Samara seemed a bit better than Jack at the biotic shield, but only because it seemed like she was able to walk faster than Jack was. I didn't try it with anyone else, and both Samara and Jack start straining at the same point. I sent Mordin back with the survivors once and Tali the second time; both survived the trip. The only squad member I saw die was Samara when I had her lead the second section's fireteam after sending Tali back to the Normandy. I reloaded and went with Miranda before the end boss, so I don't know if Mordin would have randomly died because Samara kicked the bucket.
I brought Grunt and Thane with me to teh final battle. It took me forever to beat the final boss (I think it had destroyed all but a single platform), and they both survived.
Needless to say, they were all loyal. I get the impression from other people's posts that not having everyone loyal, or losing certain people at certain times will trigger the random Mordin death. I think if you're really unprepared, other squaddies will also randomly die.

What is the factor in Gabby, kelly, and the cook dying? Everyone else made it out fine.
You've done something else after your crew was abducted. As soon as this happens you must follow them immediately if you want them to survive.

I don't think it matters how you build them, though I suspect what upgrades you research has a lot to do with the survivability of who you send through the vents and whoever's in the secondary squad. I think Shep's level might play into it as well. But that's just a hunch.

Says it right in the skill discription.
How I did it;
Sent tali in the vents
Samara as biotic
Mordin escort crew
Miranda/Garrus for the finale
All upgrades, all everything, max paragon.
Main note, is never have mordin on the battle field at the end.
I tried a plethora of things, and everytime he has to stand with the door he will die. Fully upgraded his special tech thing, everything, he still dies.
Garrus is easily the best choice to lead the second team while you do the biotc thing, if you're really paying attention. Guerilla tactics = hit and run etc, says right in the descriptions.
Miriandas skill = Says no one left behind etc, she leads initial second team, bla blah

I did. He was the only one I didn't have "Loyal" but I didn't assign him to do anything either. I had no one die.

1. I sent LEGION through the ducts. He is the best tech expert that you have, and he is synthetic, therefore not likely to suffer badly from heat exposure.
2. I sent GARRUS as the second team leader (first time around) because he is the only other person on your squad who has actually led a team successfully. (Zaeed was another choice, but the person in the duct dies every time you use him) The reason for that is simple. At the doors when the second team leader yells "give some covering fire" everyone should pile up at the door and shoot. If you are using someone without leadership skills, your squadmates won;t react in time, resulting in a death.
3. I chose JACOB to escort the crew back (OK, now here is what happened to me... The only surviving crew I had was Doctor Chakwas. I watched as Kelly got ground up into paste... I DONT KNOW why this happened, but I think you need to do the Suicide Mission as soon as it's available to help your entire crew. I did not - I went to all the planets in all the systems to do side missions.)
I had no particular reason to choose Jacob, other than he served in the Alliance so he had combat training. (I orginally chose Grunt to be escort, and that works too)
4. SAMARA as biotic shield. My reason was this: I really dont like the attitude of Jack/Zero, and Samara IMO was stronger.
5. GARRUS again as the second team leader. Same reasons as before.
On my team I had Zaeed and Thane with me the whole time, even at the Reaper battle. However, on that first runthrough (I had Grunt escort the crew/ civilians) Mordin died randomly. Luckily I had a save before the second crew choice, so I sent Jacob as an escort, and took Mordin and Zaeed with me for the final battle.
I figured that I should leave as many talented people behind as possible to help protect eachother, and still leave me with one guy (Zaeed) that can pack a punch. I also thought that Mordin's incinerate would come in handy at the Reaper fight. (It does not.)
So, if you have ALL the ship upgrades, all the ALLY upgrades (Grunt's shotgun, Legion's sniper, Jack's amp) and you use the crew list above (or maybe a similar one, I haven't tried) AND you do the final mission fast enough, you should save your whole crew, and save your allies, and get the good ol *BLONK - ACHIEVEMENT UNLOCKED*
EDIT: Make sure ALL your allies are loyal to you. There are two points in the game where you can lose loyalty, and that is a fight between Miranda and Jack, and a quarrel between Tali and Legion. If you have a high enough Paragon/Renegade power you can remain neutral and not lose their loyalty. (The one between Legion and Tali is kinda cool IMO)
