I had Garrus and Zaeed on my team for the final boss.
The second time beating the game everyone survived. All of my choices were the same except for #4. This time I had Garrus lead the second team.
I had Mordin and Sumara on my team for the final boss.

Also what dialogue option you choose before the final battle can affect it. It you say "we had better hurry" instead "of you guys cover us" people die if you don't move fast enough. Compound that with two dead people meaning less cover fire and you're lucky only one person died.

don't do loyalty missions.
Really just do what you need to to get through the Omega 4 relay and nothing more.
Also I would suggest putting the wrong people in positions in the final battle.

Oh, and the only person to have died for me was Mordin. Maybe its mandatory? O_0

Nobody died in my game before the final fight, I chose "we better hurry" and took my sweet time and still nobody died. Mind you, I was shaking like a dog ****ting razor blades thinking my whole team was dying because I chose the "hurry" option. Thankfully they all made it somehow.
I used Mordin and Grunt to fight the final fight, BTW.
Gave the apporpirate people appropirate responsibilities throughout the Collector base.

And for the entire time, i had grunt and tali with me.
we all survived. although kelly was dead when we got there. but i still got the no one left behind achievement.
and just incase, i had all the alternate costumes on.

Final team: Myself, Legion (for the super sniper rifle), and Miranda (my LI). I selected the "we'd better hurry" option and then hauled ass.
No casualties for the good guys.
Edit: And while I really don't think it matters, all my characters were in alternate costumes.

I chose the hurry option, didn't have anyone stay back to hold them off, and I had Miranda and Thane with me the entire time. Not my normal team, but I was afraid that Miranda was going to do something that would get others killed (as, like I said earlier, I didn't have her loyalty). She didn't betray me or anything though, but died in the explosion after the last boss. Just when I thought everyone would make it, too.

Sent Legion into the vents-he survived.
Miranda led my team 2 every time- she survived
Tali led the survivors back- She survived
Grunt was with me in my fire team the whole time- He died (swept away by insects as the doors closed)
Jack died unexpectedly (cut scene after the final battle) she maintained my Biotic bubble.
So that was it for me on deaths
Dont miss jack but I will miss grunt if I choose to import to 3
And for some reason I never acquired Zaeed.....maybe because I was a soldier...I dunno.

Legion in vents. (Was suspicious that this was a trap and would rather lose a machine if so.)
Garrus led both teams because I trusted his ability the most. (When Jack and Miranda had their squabble, I *almost* made Jack leader just to annoy Miranda, but I'm so glad I didn't do that.)
Jack as the biotic shield. (Picked her because she was a fierce and intense person. I felt she'd give 110% to the job.)
Everyone on the team survived (all loyal, everything upgraded), much to my surprise. I was waiting for some kind of obvious death choice like in the Kaidan/Ashley scenario. Saw that people were having issues with Mordin so I reviewed my screenshots. Turned out that he was in my team the whole time (mainly because I liked his incinerate ability). He was the one that was sliding off as my Shepard grabbed him in the end.

He is a mercenary for hire who has no problem killing the people he is working with so he gets a bigger cut of the profits. Please tell me how this translates to a good leader? I want to know your reason.
He is a grunt, he is muscle, he is not leadership material.
Even if he was, he is also a DLC character, I love many Bioware games but so far they have a crappy track record with DLC, its never 100% implemented and always feels tacked on.

Garrus lead the Fireteam.
Told jacob to escort Chakwa to the Normandy.
Chose to hold the door and selected Jack to be my forcefield lady
Garrus lead the team again.
Took Miranda and Garrus with me at the end.
Mordin died during the final battle.
I was sad to see him go. Funny guy. He thought I was hitting on him. I might get all the Tech upgrades next time.

I chose Legion as the first specialist. His dossier, refers to him as an expert hacker, and tech expert. My prior experiences with him, suggest he was even better suited for the job than Tali. I also figured he would fair better in the climate.
I chose Garrus to be leader, every opportunity. Initially, I struggled a little, between him, Zaeed, and Miranda. But it was clear Miranda would be a poor choice to lead Jack. And Zaeed would be too much of a gamble. Garrus had experience, with C-Sec, and leadership experience from his days at Omega. I remembered, how much the death of his squad, ment to him. This lead me to remember, the talk I had in Mass Effect 1, with Ash/Kaiden (it was the same every playthrough), that "I (shepard) get better for "them" (those serving under me that die). I immediately had a strong, undeniable desire to survive this mission, with everyone intact. Zaeeds goal, was to fullfill the contract. Mirandas to succeed at all costs. If I wanted them all to survive, I didn't just need a leader, I needed a leader not willing to compromise with casualties. And it was then that it was clear. Out of everyone in my squad, Garrus was the best candidate.
In the next mission, I chose Thane to escort the survivors. While deciding, I recalled how Thane went out of his way to save innocent lives,. Diverting from his mission, to kill Nassanas merc who were preparing to kill innocents. And then locking them in a safe rooms to shield them from danger. He did this more than once, during the course of the introduction mission. Further more, he was an expert at stealth. Which I reasoned would be invaluable when evacuating survivors, from a space station full of hostiles.
I chose Jack to hold the biotic shield. Her dossier, her backstory, indicates she is the greatest human biotic alive. Cereberus had an entire facility, and a dozen biotic children, just to hone her skills. While there were plenty of good biotics in the squad, she WAS the best. I chose to take Samara, and Mordin with me. Samara incase Jack began to fail. And Mordin because I reasoned he might come in handy, if we had to think of something on the fly. It never came to that. But still.
In the final run, I took Samara and Tali. First, at no point did I want Tali, Legion, Jack, and Miranda in the same place. Jack and Miranda, had shown anomosity between eachother a couple of times. Tali and Legion had argued more than once. Garrus was a great leader, I didn't know for a fact if that anomosity would show itself in anyway. But I didn't want to take the risk. Further more, I thought Talis technical skills, and Samaras biotics might come in handy. Partially in combat, partially beyond that.
In the end, no one died.

Its still highly suspected by many but not yet proven that if you take to long doing each area there is a chance some one will die as well. Mordin and Tali seem to die a lot in the final battle with no explanation, since they are the least tough character's, most suspect they are the first to go if you take to long in the fight.

All loyal and I did the mission as soon as it became available.
All known ship and equipment upgrades. (I missed alot of planets - going through them now post-end.)
Tech specialist: Legion (although I think Tali or maybe Mordin may work) Not sure if taking your time to punch those valves is a factor - I rushed all the valves, making sure Legion didn't complain more than once.
1st and 2nd team leader: Miranda (2nd time she got shot in the stomach but shrugged it off.)
(Looks like Miranda and Garrus have good leadership. Zaeed, well, he did lose control of the Blue Suns to Santiago and has been solo ever since. Maybe Grunt cos of his Warlord genes but I think it's a risk.)
Biotic: Samara (I think Jack will also work but nobody else. Basically a full Adept rather than a split class. Also, might want to double time it to minimize drain on her powers although the weakening part might just be scripted.)
Survivor Escort: Garrus
Everybody lived I think. Phew. Really weird seeing Joker provide covering fire from the airlock. Would have expected any other survivor but beats a random grunt.

I did. Everyone survived. (Legion in the ducts, Miranda on the second team, then Jack for the shield, Garrus on the diversion team. Sent Grunt back with the crew.) I just didn't use him for anything...frankly I wouldn't use him for anything anyway. He's the onyl character I really don't like all that much, and it's mostly to do with the fact that he's so freaking shallow. *sigh*
For kicks, put Mordin on the diversion team in the second part. His death (being shot by in the chest through the gap in the doors) is a priceless gaming moment. More so if you talked to him about Kirrihe.
As for the vents, no matter who you send in, they'll die if you don't pick the right leader for the second team. For example, if you chose Samara to lead the other team, despite the fact that Miranda calls her an excellent choice, you'll hear updates on the radio that they're falling behind and will try to catch up, yadda yadda. (Oh, nice job BW on that one. Make us think we made a good choice and then kill-off someone. Well done.) Miranda, Garrus, or Jacob will push forward and make good time. I would think that the justification for this is that if they show up late, your tech is taxed too heavily and can't work so quickly, resulting in a door jam and "OH SHI-" moment.
Similarly, someone on your squad dies if your biotic specialist isn't loyal or up to the task.
IMO, this adds a real sense of realism to the mission. If someone isn't commited, it's not necessarily them who pays for their hesitation, inability to perform, or lack of trust. Someone else tends to die. That's a lesson that's taught to all professional military personnel; your screw-up might not cost you your life, but the life of the guy next to you.
Regarding "random" Mordin deaths: It's the leaders. The common link in all of these is that neither Miranda nor Garrus nor Jacob are left to coordinate the defense of your rear guard (unless I missed something. Maybe you need at least two of them in the rear guard; that is a pretty substantial group, after all). You have a bunch of people who aren't necessarily knowledgeable with small unit tactics with no clear leader. Simple.

Tech specialist: Legion. Seemed logical, being a geth and all.. And in one of his conversations he said something about calculating stuff at lightspeed or whatever, so I went for him.
1st team leader: Miranda. I didn't know who else to pick. She seemed to know her stuff and always had this "bossy" feel to her, and therefore she seemed way better suited for the job than Jack, and since those two were arguing, I figured that one of those must be the right choice, so it must be Miranda.
Escort: Grunt. I figured, if they would get attacked, Grunt was a good tank to keep Chakwas safe (I did too many sidequest, so the rest of the crew died before I got there)
Biotic barrier: Samara. She was always so dedicated and disciplined, and I knew she was a powerful biotic, even more so than Jack or any other crew member imo, so I chose her. Jack always seemed too chaotic, and I think the rest wasn't powerful enough.
2nd team leader: I suddenly realized that Garrus lead a whole team for a long while, and he probably learned from his mistakes, so I chose him for this job. I probably could've chosen Miranda again, but I like having her on my team.
Final team: Miranda and Thane
Everyone survived.
