I only have Varric, Cassandra and Solas with me.. am I doing something wrong? I went and created 6 camps, killed some mages and templars, talked to the horse guy about getting some horses, killed bandits, sealed a couple of rifts... and I still have a bunch of active missions/quests, but I cant figure out how/where/when (and in what order) to recruit the remaining companions, can anyone point me in the right direction?

You need to complete quest "The Threat Remains" for first recruitment missions to appear.

And after I do that the recruitment missions will just appear at Haven? Or will I need to go somewhere in particular to trigger them?

Two actually start at Val Royeaux, others at Haven. Well, with two exceptions, but all of them are hard to miss.

After completing The Threat Remains, four recruitment possibilities with arise. 

Blackwall: Leliana will mention him in a cutscene and you go find him in the Hinterlands. 

The Iron Bull: One of his mercenaries will be at Haven and talk to you about them wanting to work with you. You go to the Storm Coast and recruit him. 

Sera: In Val Royeaux, an arrow will land near you and you have to do a hunt for notes in the city. Eventually it will unlock a small map area on the World Map where you'll get her. 

Vivienne: There will be a man with an invitation for you. It unlocks a small map area on the World Map and you go there to recruit her.

You will get some of the recruitment missions in Val Royeaux and some will be at Haven. Don't worry, you won't miss them.

Wander around in Val Royeaux and an arrow with a message will land at your feet. Interacting with it starts Sera's recuritment quest.

 

After you've completed Val Royeaux, a duke's estate will appear on your map where you can fast travel to, this is where you recurit Vivienne.

 

Leliana will inform you about the Grey Wardens, that's when you learn the location where you can meet Blackwall.

 

A mercenary will approach you after returning to Haven, this starts Iron Bull's recuritment quest.

 

Cole and Dorian come later, during main quests.

Thanks to everyone who answered! I was actually worried I would miss some of them as it happened with Isabela in my first da2 playthrough when I noticed too late I hadnt recruited her on time...

Yes! I finally got everyone except for Dorian and Cole, who will join me later if I understand correctly. And I finally left the Hinterlands too ahah
