While it is true that Meredith rejected Alric's insane "tranquil solution", (eliminating every mage by subjecting them all to Tranquility) is it plausible that she didn't know that Alric was using Tranquility punitively on mages who had passed their Harrowing but somehow stepped out of line?

I can believe that Meredith was unaware of Alrik's abuse of Tranquility.

 

Of course, his actions were probably facilitated by Meredith herself abusing the rite and using it punitively.

It's possible but unlikely. Anders tells you that 12 mages have been made tranquil the first year, and Meredith held a tight rein over her Templars early on.

 

You could say after the deep road trip by Hawke and the lyrium was sold to her it's more likely, but I have my doubts even so.

It's possible that she knew about Alrik's activities, but the moment it got out she claimed that she didn't allow it. 

 

Of course, there is a chance Alrik secretly rebelled against her.

This letter confirms that, at least, Alrik was trying to conceal his activities from Meredith.

I'm guessing that Meredith authorized the punitive use of Tranquility in violation of Chantry law, but Alrik was using it for even the smallest infractions, which probably wasn't what she had in mind.

And then, threw a templar out of the Order for passing the letters.

She threw a drug addict on the street already too addicted to the lyrium to control his urges instead of giving him an early retirement, so he could be treated by Healers.

I think she knew, I also think she planned with Alrik that he be the one to deal with Templar malcontents, that way she had plausible deniability if the process was ever discovered by the Chantry or the citizens of Kirkwall.

Regardless of whether she knows or not she failed her duty. The Templars exist, in part, to protect mages as well as the populace at large. If that is so- then using tranquility which is a "final solution" that should only be used in the direst of cases- it would seem reasonable to think that she is aware of every rite of Tranquility that happens under her purview- or someone close to her should have alerted her "hey boss did you know there are now 100 new Tranquils, hmmm what's up with that". 

 

So she was either keeping her hateful eyes on every move the mages made...or not. 

 

So while I do not have a citation that says "Hi Meredith remember when we made a bunch of tranq mages! heehehhehahha" I do not require it. 

 

She failed to do her job properly. 

 

And IDGAF about the red lyrium either- she was hell bent on power far before that. Additionally what truly Maker fearing religious devotee would think some strange new lyrium is a grand idea? Based on all commentary about it in DA2 and DAI- you could say it gave off the strong vibes of bad juju to anyone near it. That Meredith was so hell bent on any and all advantages to smother dissent and accrue power may have left her weak to not sensing it or not even caring. 

 

Basically all humans are equal to temptations of evil with our without demons as provided amply by many Templar acts and behaviors (not that killing innocents is something I agree with whatsoever, nor do I despise all Templars).

She probably knew that he made mages tranquil and probably agreed on the rite for certain mages who were actually trying to break the law like Karl. However for other victims like the female mage in the gallows whose only crime seemed to be that she didn't want to sleep with Alrik he probably told Meredith some fake bullsh*t story about blood magic or something and didn't outright admit that he just wanted to bang her but she refused. And Meredith probably didn't care enough to verify those before allowing the rite.
