I've searched on YouTube for this yet have never come across it. It is possible, right? Is his death scene different without Kolyat (presumably) there? I'm also assuming he has no Citadel DLC content.
Disloyal Thane in ME3?

Pretty sure if Thane isn't loyal he doesn't make it out of the end of ME2. And if he does survive due to no loyalty to Shepard there is no reason to contact him post mission.

I was researching this last week. I found ppl saying that he has a couple of different lines in the hospital as he couldn't stop Kolyat, but other than that everything else plays out the same way.

Update: I was researching the suicide mission specifics and Thane can indeed survive it if disloyal. I just strangely can't find any video evidence of it.

Surprisingly, none of my recorded playthroughs have a disloyal Thane in ME3. Generally, I fail Thane's mission when I intend to kill him. I know I have done it once and his initial dialogue in the hospital is much darker than if he is loyal. I do not remember if Kolyat still shows up during his death scene, but I think he does.

If I wasn't determined to get everyone's loyalty, I'd like to fail Thane's loyalty mission just to stick it to him because of his moral hypocrisy that irritates me. I'd also want to fail Tali's loyalty mission by turning over the evidence to the quarians because I believe it's the right thing to do.
On my first playthrough I actually did fail Thane's mission, but he died going through the Omega 4 relay because I didn't upgrade the ship at all.
Lastly, Kolyat showing up in ME3 makes no sense; even less if he shows up in the Citadel DLC.

Well, now I have a new mission for my current playthrough!
I originally intended to kill Thane during the SM. With Kirahee dead as well, this would have led to the scene where Udina presents the doctored evidence to the VS. Now, however, I think I may keep Thane into ME3 just so I can get a recorded version of that. I will need to look into it, however, since I wanted two casualties during the hold the line segment. I will need to check the priority order for deaths during that segment and see if he can survive as the third disloyal member.
Update: well, it looks like Thane is far enough down in the priority order that I can have him disloyal and still kill two "hold the line" team members while having him survive. Just need to get the math right.

Ah, messing around with the SM maths so I can complete my homicidal plans....fun times. ![]()

That's one thing I've never done. Is fail his loyalty mission. But then again. I rarely do his loyalty mission since it makes no difference. When I play ME2, I usually recruit him after the suicide mission. I only use him to avoid the councilor from been killed. His dialogue when I see him in the hospital is slightly different and his son's dialogue is slightly different when seeing him before Thane dies.
I'm tempted to do a playthrough failing his loyalty mission to hear the difference in ME3, but I'll wait to read the result from someone else.

Yeah, it's technically possible for anyone to survive if disloyal, depending on the choices you make during the mission. You just need to make sure the hold-the-line group has a very high total score, because being disloyal doesn't guarantee death in that scene, it just gives characters death priority if the score lands below specific thresholds. A disloyal party member taken into the final battle, made a squad leader, sent to hack the gates open, put on barrier duty, or sent to escort the crew back to the Normandy will always, always die, however. Keeping disloyal squadmades back to hold off the Collectors is the only way they have a chance of living.
Naturally, it's easier to keep disloyal squadies alive if you recruit all squadmates and have both Zaeed and Kasumi on the Suicide Mission along with the basegame squadies, since the more people you have, the higher the score.

Kasumi will lower your score even if she is loyal. Zaeed will raise your score even if he is disloyal.

I recently did the Citadel DLC with a disloyal Thane where Kolyat succeeded in his hit. Spoiler: It is exactly the same. I get few players would have this outcome, but this is just stupid. I would rather Kolyat not show up at all in the DLC than this awkwardness about Thane stopping him, which never happened.
The DLC should've revealed more about Thane at least.
Kolyat: Remember that time I killed a turian politician.
Shepard: Right. What was his name?
Kolyat: Jor-something.
Shepard: Wait, I thought you drell had like perfect memories or something.
Kolyat: Oh did father do those freakout things where he just blurts out something from the past? Yeah…that's... not a drell thing. Just FYI.

I did a disloyal Thane and kept him alive into ME3. The only real difference is that he is much more negative when you speak to him in the hospital. If he's loyal, he has a good outlook on his life and speaks of the positive changes Kolyat in his life. If he's disloyal, he's much more darker and negative because he couldn't stop his son. But his death scene plays out pretty much the same. Kolyat appears out of nowhere to say goodbye to him.

That depends on who is loyal and not loyal. To have everyone surivive, at least 7 have to be loyal. Here's the details.
If you want to know more about the suicide mission, here's a thread for that mission.
