I just started a new game with a world state where Hawke is a mage, and when I asked Varric what happened to Hawke's friends, he mentioned a living Bethany instead of Carver.

What the hell? Is that a common glitch?

I even went to the Keep to check my choices again and it was there. Warden Carver and dead Bethany. And I have only one world state, so there's no mistake here.

I've played 10 or so games with all but one of them having the same world state, and I've never had that bug happen to me.

My only Hawke is a mage and I've played over a dozen times and never had that happen.

I even reloaded the save file and talked to him again. I'm like "Really? Bethany is alive... and helping the Circle?"

Is it possible your custom world state didn't import and you got the default world state?  Is anything else off?

I know it worked because when I asked Leliana about the HoF, her dialogue showed that he is male and alive.

Afaik, default world state has mage Hawke, so it still should be Carver.

 

OP, I played with mage Hawke many times, but never encountered this problem. I'm sorry I can't help.

The only glitch I had with my Hawke import was when my rogue Hawke turned into a warrior. At the very least, I can confirm that there are potential import bugs

That must be it. My mage Hawke turned into warrior Hawke.
