I had TONS of stupid deaths in ME2, The worst by far.. Is the one on the collector ship, Shepard LITERALLY just started floating... I was exposed to enemy fire and died in an insant...And its also annoying when you run for cover but shepard grabs cover infront of the enemy and you die
I gotta add a death that just happend to me, I tried to do a strafing test with the locust SMG, I strafed 2 LOKI's and got fokd
Probably one in my last run, also during the Collector Ship. I had thought I had defeated the last wave of Collectors on the platforms and stood there without cover. Got a nice surprise from behind (I'm sure they spawned after I took down the last Scion, but I really didn't pay attention at all) 
I have issues with floating Shepard as well, so far it luckily hasn't killed me though. I also had some deaths due to weird camera angles even though I was in cover.
When Shep gets stuck on a high point/ledge. There's a ledge in Grunt's loyalty mission that you and your squad mates can run along, next to one of the ramps, but Shep can't get back down again. And another on Grunt's recruitment mission when you're running down a ramp towards the bridges full of Krogan. So I get stuck, enemies turn Shep into a human colander, and I rage at the screen for a while. 
If Andromeda doesn't have a jump button I'm gonna lose it...

I think my stupidest death(s) all come from being overconfident in my own abilities sometimes, or from pushing my limits too far (usually trying to cast that one power while at half health thinking i can just survive... which i don't).
But i think my stupidest death came from trying to emulate some videos on saw on YouTube where a couple of Vanguard players on Tali's recruitment mission would just charge the Colossus straight away and take it out in pure awesomeness. but whenever i try it.... "Live like a Vanguard, die like a Vanguard".

You are not alone my friend, Sometimes I just get super greedy and attack an enemy with half my healthbar gone
I have never been floated when an enemy is nearby, which meant I was potentially stuck forever without a reload.
I have had many deaths resulting from ill-advised charges, especially way back in the day. Had one where I passed out during the Suicide Mission so that was probably stupidest from a real world perspective.
probably during Garrus' mission. Got flashbanged and exposed and every bullets shave my shield before I could find cover and then some bloody varren show up from nowhere.

Not a death, but sometimes my keys get stuck (something I need to fix...), and I failed Thane's loyalty mission because instead of updating Thane, I was continually walking into a door.
Just admit it.
Shepard failed Thane's LM because she was watching that hot Techno Turian dance.

Sadly, for me, you all know the death. It is not glorious nor fabulous. Garrus' recruitment mission, a certain Vorcha Pyro. Ya, *THAT* pyro.
Mine actually happened yesterday. I'm on my second run through the trilogy and just started ME2 yesterday-I was still having some trouble adapting to the controls after a quick change from ME to ME2 so instead of taking cover, I just ran at a crate assuming I would automatically take cover as it was in ME.
Result-the mechs on freedom's progress can be quite merciless if you don't duck when they shoot
