I was flipping through my achievements in Origin and noticed that, even a year-and-a-half later, this achievement is still gray.  Not only did I kill all 10 Dragons on my very first play-through, but I've slain several more, since.

If you're disconnected at the moment you drop the last dragon, the achievement doesn't flag as complete.

On the off-chance, I'd also check your Options -> Account settings in DAI as well as making sure you're not running Origin in Offline Mode.

 

Edit: I'd also try reloading your last save of the character which killed all 10 dragons, just making sure you're connected.

I fired up a save and killed the Crestwood dragon twice. That did it.
