Now this I understand. It's puzzling too because EA's goal is to sell to a wider market and a M+ rating will remove the 17yr old teenagers. I was also unaware that many retailers would refuse to sell AO games. However, this may be mitigated by digital sales.
Could be a typo
Has anyone seen a M only rating for Mass Effect Andromeda? please provide a link.
I expect this to change as digital distribution becomes ubiquitous.
As others have said the console makers won't allow an AO game on their platforms and Twitch won't allow streaming of AO games either.
EA is also a publicly traded company, making a game AO could be considered by their investors a waste of money and then face lawsuits over the fact it cost them money, for their investors sued over Battlefield 4 and its launch issues.
As others have said you really need to try to make a game AO I can only think of one game that has ever been rated AO due to violence and that was Hatred. The rest also have nudity or strong sexual content also part of the warning label.
Here is a list of the games that have been rated AO by the ESRB.

Edit: I thought Steam didn't sell AO games, but they did sell Hatred
