Not much really, they just added lots of better weapons and made the characters more powerful.

I completed an Insanity play through recently and with a few games of MP to go along with all the other crap you gather, it said my GR was 21,000+ 
It went back down to normal after a day

It's funny how that galactic readiness drops when you don't play. It makes you feel like, these armies can't do anything without you....*ahem*...."ok guys we are at 100% readiness...i'm gonna go take a break, smash a few asari's and I will be back in a few days"...them: "yes sir..we are good..no worries".....*comes back a few days later, readiness at 65%*....WTF?...agghhh....Aw hell naw...this is some bulls**t......
