So... I did the mission on benning and it all went good no problems at all, Just randomly in the middle of the mission, I know i had full health because i just used adrenaline burst, SHEPARD RANDOMLY RAGDOLLED AND critical mission failure? dafuq?

what level of difficulty are you playing? On Insanity, grenades usually means instant death.
