I created a world state where the ashes of Andraste were poisoned with dragon's blood and Leliana was killed, but every time I load a new game of inquisition she walks in with Cassandra, alive and well.

Either this world state isn't loading correctly or there's some other kind of error going on.

If I get to the part where I ask her about the Hero of Fereldan, does she say that he/she desecrated the ashes and left Leliana for dead?

These are all retcons and mistake made from Bioware since DAO

Leliana is not the only case there is also Oghren,Anders and  even others worst retcons.

No, that is canon. Even if killed, Leliana returned to life.

Spoiler

And yes, she will comment on her death at the Warden's hands.

Yeah, the Anders one was annoying when the awakenings epilogue said that he stayed with the wardens for years, apart from an unexplained absence of a few months.  Maybe they didn't realise the plot for DA:2 was going to happen over the course of years?  What happened with Oghren, though? I missed that :o

Ah, well at least it's not a retcon. RETroactive CONtinuity is where the previous canon story is changed from how it was originally presented, usually in a flashback or a whole re-write. Accounting for it with a doppelganger is fine by me. I don't mind the spoiler, I guess I'd have needed it or I would have kept trying to alter the world state every time she appeared on screen after changing it so that the warden killed her.

Regarding Anders, it was not originally known whether they would bring forward Velanna OR Anders for the Justice merge, each were under consideration at one time, so that may account for some of it.

If you kill Oghren in DAO he will still be present as a companion in DAA

Would that be when you have to choose between Branka and Caridin? Because I sided with Caridin and Oghren was OK with it

No, Oghren will eventually attack you if you lower his approval enough. This is fairly difficult to pull off, though. It requires a lot of dickery and/or the use of Feast Day pranks.
