Most other staff's and such are named stupid names like Blizzard, Dragon's Breath, Firestorm, etc.

I have a staff that does lightning base damage and Chain Lightning damage in the Masterwork part of it. Its name is Ride the Lightning

No, I think the worst is what I do when I'm in a hurry to get full new sets for everyone, then I end up with (Adaar is Inquisitor in this example.)

No two initials are the same class so, it works. I then do the same for head wear, upgrades and weapons so each ends up wearing a mess of duplicate letters LOL.

On one playthrough I named all the crafted gear and weapons after Westerns - The Good, The Bad and The Ugly, High Plains Drifter, Lonesome Dove etc. I even had one item called Paint Your Wagon by the end
. On my current run I keep forgetting to name them, but Gwen does have a high crit chance bow called Everyone's A Critic.

I took staff upgrades - blades and shafts - that had slots for leather that added to critical hit % chance. Then used great bear. I also think I had critical hit % chance rings on him. All three slots on the WoL schematic were done in great bear hide. I can't remember if his armor also allowed for critical hit slots or not. But if it did there was great bear added to it too.
Edited to add: I will say that this was a very specific build aiming to get his critical hit chance as high as humanly possible. All the hits were critical hits! I sacrificed other stats to get it that high. ![]()
I tend not to get too creative. I am really good at naming things but it does come and good and I usually end up calling my companion's items really pathetic names. Like the handsome prince for Dorian's armor, or Cass's Swiss Army Knife, for a sword that did a little bit of everything. Or Varric's Exo Suit, now that was a fun one, it was named because I gave him the Revered medium armor...and it looked like an exo suit.
But for my Inquisitor, Kara at least, gets the best names. Her first playthrough I gave her weapons the names of...well crafted two Dragonbows, one named Dumat's Bane, and the other Dumat's Wrath. Kara really loved mocking Corypheus. And Dumat's Bane was her Dragon Hunting bow, and her regular bow was Dumat's Wrath. It would really ****** Cory off. For Kara's second go since A. I wasn't really going for the whole '****** Cory off' thing and since I was crafting only one Bow, one with a fire rune, I ended up naming it 'Dawnbringer'.
I remembered that I named my human mage's weapon "Tempest" and her armor "Eye of the Storm", because I maxed out the storm tree of spells with her.
I really don't do much in renaming the items in this game. Fallout 4 on the other hand I rename so I don't lose track of them in the inventory.
I usually give name's with the name of the person using it attached - so I don't confuse who is using what. Dorian's Revenge, Solas' Sorrow, Bull's Wrath, Cassandra's Secret Dream - things like that.
I used to do that a lot, but I went with Inner Peace for Dorian, Elf n' Proud for Solas and Agh! for Cassandra xD.
I also had a phase of renaming things after deities, like weapons called Amaterasu's Blade, Quetzalcoatl's Rain Stick, Izanami's Sorrow and stuff like that... Then I went through a colour phase and then I used names of Billy Joel's songs, Metallica's songs and now I'm on a phase of food; my current greatsword is called Pizza Cutter.
I don't rename things, can't be bothered.
I did see on Tumblr ages ago somebody who had crafted a full set of armor made of nugskin and called it "Leliana greatly disapproves".
I found it hilarious.

Yes, mostly because it's easy to forget which one was the modified one... I name my guns after girls I either dated, my sisters or cows my father had in our farm (not that they relate, they just happen to be funny names for a weapon) I currently go with Ramona (laser rifle), Annie (pipe pistol), Clementine (combat rifle), Cassie (shotgun), Ivana (sniper rifle), Keiko (alien gun), and always name my best gun (currently an OP af plasma rifle) after me wife Shizuka, to remind me that she's gonna kill me if I don't have dinner ready before she arrives from work xD.
I named my weapons and armors in my first two playthroughs but i have no recollection what those names are it is just to long ago. And after that i got really lazy and did not name any of my weapons or armors ever again.

i like making armor for all of them for final battle and will name it their full names.
Don't think that leaving the default name is the worst. What I do is the worst. I do change the names, but give them totally functional, nerdy names like "Tier 3 Fire proc Veilstrike" for a staff.

My mage Inquisitor had imperial ambitions. So I named my weapon "Sceptre of the New Empire" and my armor "Vestment of the New Empire". Sadly, Trespasser put a sudden and very much resented end to that.
I made a bow for Sera that had hidden blades with 5 hits that I called Sera's Many Stingers. I was caught between that and "Say Wot". Shoulda gone with the latter.
