Yes she did, at least when going into a vacuum or hazardous enviornments. The lore states that shields and biotics do not protect people from enviornmental hazards, so Miranda and any character not in a sealed suit would be killed multiple times per game because of their clothing. Mass Effect 1 knew this, so every squad member had a fully sealed suit when the situation required it.
I was talking about regular tactical combat, not space-faring combat whilst on non-adaptable planets or in space. I would like to see all characters in sealed suits in those situations, doesn't mean it has to be a bulky pillowsuit either. It can be a tight fitting suit copacetic to their regular armor.

Well, Garrus did survive a rocket to the face, so I would say that is point in favor OF armor.
Kai Leng I can tolerate, for he actually DID move as he had less armor and tried to avoid frontal attacks.
Garrus can die at the end of ME2 by one bullet to the stomach. Kai Leng wasn't too tactical about frontal attacks when Shepard kabab'ed him.
i guess you mean that they NOT are noob off the the street? That makes it even more ridiculous, not less. If they have training and experience they know that clothes does very little against bullets and other objects that can hit you in battle. If the game wanted me to believe that they was dressed that way because the way they fighted, they should had made them act that way.
Whoops. I did mean not!
Fixed that. Kinectic Barriers protect against mass accelerated weapons and other projectiles. That alone is pretty good "armor" in and of itself and coupled with a biotic barriers not only doing the same thing, but also making their shields stronger--I would say they have as good of a chance as anyone else who wore heavy armor/shields.**
The tight leather "catsuits" can definitely be reinforced with ballistic weaves and whatever else that's used in the universe. Along with a shield/barrier combo, they are well protected. Samara's cleavage isn't going to be the cause of her death. As in, if something is so powerful that can bypass a shield and biotic barrier, I highly doubt a chest plate is going to make much a difference in survival. It would be like wearing safety goggles during an explosion. It would be the same if you're just wearing heavy armor with shields--if something can bypass your shields and armor, you're dead.
That someone die because they are caught off guard by ambush/having bad luck has no relevance on the topic.
I'd say it does. if your shields are down, you're vulnerable at best, caught off guard at worst. Garrus was wearing heavy armor and got caught off-guard and was hit with one bullet and died. So, if by chance Miranda has her biotic barrier interrupted and shields axed at the same time, I doubt her wearing heavy armor would have saved her. Didn't save Garrus.
But as I said, I don't consider anyone's armor, save Jack's as particularly revealing. I don't care either way, if a revealing armor was released, I most likely wouldn't wear it and I don't care if someone else does. With every companion, they eventually released thicker and more "practical" looking armors and there is no reason to believe they wouldn't again. In fact, I would like to see them do that in-game. That is what I want. I want you to have a choice of more "practical" looking armor and I want to have an option for skin-fitting armors. And if they brought back ME1's armor, this game would be a Day-one preorder midnight pick-up for me.
**I know Shields and Barriers will not protect against hazardous materials and radiation, etc.
