I just imported my DAO character to the keep. Non of the choices (literally none) I made in my playthrough appear to be correct. Is it true that keep does actually not import the decisions you made in the game and that you actually have to manually select all of them and if so, how am I meant to remember every decision I made in game I have not played in nearly 6 years?

It's true. The Keep takes only basic information about your hero from DAO and DA2 - name, gender, class and origin (in case of DAO), as well as small avatar of your character. You have to manually set your choices.

Yea seems to be so. I was going to use the my story tab from my character as a rough guide to help, however when clicking on it the tab is empty. Any suggestions?

Try your best? A lot of the choices stand out in my memory pretty well, any others, google can help with the others.

If you've got the old save game, you can go in and check your journal for some of them.

There's an on-line save reader (PC only) for DAO, which spits out a list of most choices the player made in the game: Dragon Age Save Game Tool

Although be aware that the saves can be inaccurate. It once said that I sacrificed Isolde to get into the fade
