Do you think companions (at the very least) and or our antagonists will say our first names, instead of Pathfinder or Ryder?

I almost never pick serious names for my characters. Ever. As such, Codsworth has never said my name.

You didn't pick "a**face"? There is a lists of names Codsworth would say and some of them are hilarious.

@OP, I would love it if they implemented the same system into MEA.  But to save on costs they can have your fLI call you by their first name, like how only Codsworth would call your character by name. That would be nice.

I honestly liked what they did with James and his nicknames, it not only helped him stand out but it felt a lot more natural to have someone close to you call you by something other than "Commander" or "Shepard" for a change.

Maybe there can be a small list of nicknames some companions can call us depending on some of the decisions we make? Adds a personal touch to it.

Nah, a last name or title works just fine by me. Heck, I'm one of those roleplaying dorks that think about the name for my PC and how it'll be a reflection of their personality. If I pick a light and airy name like Aspen, then I'd be playing the PC with more of a paragon and/or "good guy" outlook. If I pick a darker name like Lilith, then I'd be playing that PC with more of a renegade and/or "anti-hero" attitude.

Cause 100% of the dialogue is just colour announcing and saying a last and/or first name?

And? I'm talking PS1 bruh. That's nearly two decades ago. It ain't that complicated. You implement an archive of recognizable names like "Williams, Johnson, Alenko, Jackson, Jordan" I'm not saying every name in the book, just common ones.

Did you honestly expect them to account for a special snowflake name like Kierro?

Don't need to be a jerk about it, but with so many so many names, fake or uncommon. I kinda hoped they would of. Because when I saw the original name list, Kierran was a choice, though it got removed due to recording issues, so I hoped Kierro would of shown.

what if when someone is about to say your name something happens, like a train passing or Saren jr. attacking the ark with some geths?

No, I don't think they'll say your first name, but it doesn't bother me. What is kind of funny is that everyone calls you by your last name, but almost everyone else is exclusively called by their first name in most instances. It would make more sense if other people were called by their last names more often, so it seemed like that was just the norm. But, overall, it's not a huge issue for me.

I wouldn't want it, especially if it's only done by one character like in Fallout 4, and I doubt that it would happen.

I don't think they'll call us by our name. Contrary to most, I'm fine with that.
