Sorry, I don't know much about motor neurone disease or how it would affect what games you'd play, but I gather from your post that you want games with real time combat that don't require extreme precision.
Maybe Fallout 4? It's an FPS but it's quite easy, you can have followers who help you quite a lot in combat, and you can pause the combat any time (I know you don't like tactical mode, you can use it more as a way of detecting enemies, or making sure you're aiming at them. You don't have to move around too much in that mode selecting exactly what you want to hit). It's basically 'Oblivion with guns'. If you can do shooters, maybe you should check out the Deus Ex games, or even Bioshock.
The witcher 3 is similar to Dragon Age and has real time combat. I find it a bit difficult, but you might enjoy it.
Mass Effect is basically a sci-fi Dragon Age with shooting, it's also very welcoming on easier difficulties. It even has a setting for people who don't enjoy/struggle with combat. vYou say you got the trilogy, make sure you get the citadel DLC for ME3, so much fun - but only do it as the last thing before the final mission.
If you like JRPGs, pretty much every Final Fantasy is on Steam. Also, Zelda games are very good - even if they aren't particularly mature or have that deep fantasy worlds, and they aren't on PC (cough cough dolphin emulator) Tales games, and Ni No Kuni or even Valkyria Chronicles might be enjoyable if you like JRPGs - but they can seem a little cutesy.
Some people swear by Kingdoms of Amalur, I never thought it was anything special but if you like the look of it I hear it has a lot of game for your money.
Dragon's Dogma is a decent, if unpolished game which was ported to Steam not too long ago.
Have you played Elder Scroll online? Might be worth checking out - I hear it has a good playerbase on PS4. Also Final Fantasy 14: ARR is a good MMO.
If you're generally looking for a game with a good story then the Walking Dead or The Wolf Among Us are pretty good.
