Like movies, games aren't actually constructed in chronology. Apparently, In Your Heart Shall Burn was the last main quest added, following a decision where that Inquisitor wasn't going to be Inquisitor from the start of the game (see e.g. some earlier promo materials were Cassandra is a lot more mistrusting of you). DA:I sounds like it had a bit of a troubled development story-wise, like DA:O before it.
This is why IYHSB also is so well done, with crisper cutscenes, a tighter narrative, etc. It's really a product of a team that has a far better grasp on their product.
