I saw some old threads talking about the game getting stuck in 5.1 surround mode with dialogue only going to center, but the solutions for that ranged from "use headphones," to "make sure Windows is set to stereo or quad." I have been using headphones so that wasn't it anyway.
Using onboard sound on this machine, Realtek ALC892. Win 10. Hardware Sound Enabled, although I tried it with software too. I think I currently have dialogue set to 100%. Sound Effects to 65% and Music to 70 or so. Like I said, volume of dialogue in cutscenes is more than fine, it is also mostly fine in the Mako. Walking around they are quiet.
I am pretty sure the Dialogue slider does effect this speech when exploring / in combat. In the Virmire camp when you can talk to everyone, I set it to 10% and practically couldn't hear them at all.
Noticed that in Engine.ini there is a section that seems like it should control volume for different things. It is under [ISACTAudio.ISACTAudioDevice]

Looks like that is where some volume control is. I tried searching for some of these variables but didn't find much. Anybody know if one of these is for the dialogue I am talking about?
