Just been reviewing my Keep & I've noticed that some decisions, despite having been chosen multiple times, have not unlocked, for example, I can choose that Bull sacrificed the Chargers, but I cannot choose that he was loyal to the Qun, that option remains hidden, any ideas?
Not all of my Inquisition choices have unlocked for me

That's the Keep for you. Some choices don't want to unlock no matter how many times you choose them, some unlock even if you didn't choose them, some unlock uncorrectly. I would suggest switching spoilers off, so you have all choices visible.
