 First mission after meeting the Illusive Man for the first time.
I walked through the first Mech ambush no problem. On the second all Mechs were destroyed Except for the Faceplate outline of a FENRIS Mech. Jacob and Miranda just kept attacking it. I could not move on.

I had the same problem with the drones or whatever those flying robots are called. I killed 2 of them and the 3rd vanished. My team was still attacking the red square box that was flying all around but it couldnt be killed and I had to restart the mission.

reload corrected the issue, still annoying.
