I don't get why everyone uses Legion :/. Hes as likely to survive as tali, so I assume its RP reasons. And why on earth would you use a Geth construct to operate solo, in which his success determines the entire gate of a mission, a geth you've just acquired.

Entire team and I survived, some of the crew got killed including Kelly because I did Legion's Loyalty quest after the reaper IFF. No biggie, as long as my sexed-up Tali made it out Image IPB

FIrst off, I figured a synthetic being would be pretty adept at that. Not the most ridiculous conclusion. Also, I recruited and gain Legion's loyalty, same as all the others. Are you speaking from anti-Synthetic prejudice?

Survived it first (and so far only) time I did the ending.

Team 1 leader: Garrus
Has exceptional experience leading a team

Team 1 Specialist: Tali
The ultimate tech specialist

Crew Escort: Mordin
Has been in Special Forces, but moreover is an extremely capable medic, so can quickly patch people up

Team 2 leader: Garrus
See earlier comment

Team 2 Specialist: Samara
Biotics as strong as a matriarch (which in-game means nowhere near my power, but you get the ideaB)), and not as unpredictable as Jack by far

Boss fight: Thane for long range damage, and Grunt for close range damage and defense.

EDIT: Obviously everyone was loyal to me. Full paragon playthrough (obviously some Renegade points because of bugs), lvl 23 at the moment of completion.

I finished today with all crew members and party members intact.  Here were my choices:

1st part:
Tech Specialist - Legion
Fire Team lead - Miranda
My party - Tali/Jacob

2nd part:
Escort - Zaeed
Biotic Specialist - Samara
Fire Team lead - Garrus (I picked Grunt the first time and he died after that part, so retried with Garrus and he made it)
My party - Miranda/Tali

I had the loyalty of everyone.

Door Opener: Tali

I almost roasted her, but she survived.

Strike Leader: Garrus

I figured he had ample experience in the combat and leadership department, and he was loyal. He was also my romantic interest.

Biotic Protector: Samara

She was my favorite of the biotic characters.

Escort Guard: Thane, made it out fine and without incident. Him being an assassin, I thought the job fitting for him since you're trying to sneak unarmed people back to safety.

Division Team Leader: Garrus

Just like before. Garrus is my favorite character so I gave him the responsibility.

My team for the final fight was Garrus and Mordin. Everyone lived, although apparently I didn't rescue all of my crew - only the doctor - having not realized that dallying about before hitting the Omega 4 relay kills them off. Damn. Guess I have to play through again.

I think matriarchs are more powerful than Shepherd. You didn't exactly face a matriarch one on one. Besides, it would be so cheesy for Shepherd to be like the best at everything. Bad writing. No Biotic on the squad should be as powerful as Samara, save MAYBE Jack. Both should be way stronger than even Adept Shepherd.

You got somethin' against Jack?

Survived along with whole squad and all but one of the crew.

Tech guy: Legion, strong techie and thought he would handle the environment better being synthetic.
B-team leader: Garrus, experienced and trusted
Fireteam: Tali and thane

Biotic shielder: Samara, extremely powerful and has the discipline and experience to maintain such a shield under fire and for a long time.
B-team leader: Garrus, ain't broke don't fix it
Fireteam: Grunt and Jack
Crew escort: Zaeed

Took tali and thane to the final encounter.

Had all combat ship upgrades, most or all squaddie specific upgrades, and all but Miranda were loyal.

I wouldn't entrust Zaeed for the crew escort as i had bad experienced with him during doing his mission, he would rather let all the people die just to kill his enemies, i think the crew escort's best candidate would be mordin, seeing how he would be able to help so many people in his clinic during the blue sun and vorcha raid...

What I don't get. I did all loyalty missions, did all the upgrades. Now, I did take my time going through the relay after Legion's quest resulting on the crew dying but Chakwas. Legion on Ducts(died), Samara as first strike leader(nobody died), Jack as Biotic Spec(nobody Died), Samara as Leader again(Samara and Mordin died), Zaheed as Escort(nobody died). But still, I everyone was Loyal, upgrades done, how come I still lost three members? Is there some random quality into deciding who lives and dies?

Survived and so did everyone else. Was worried for a second I was going to lose Tali when she was in my part for the final boss...that would have sucked since she was my LI in the game. Damn you and your dramatic cut scenes Bioware, lol.  Oh and I used Miranda second team lead, Samara as the biotic, Legion as the tube guy and Garrus as the escort and had no issues.

I managed to keep everbody alive using:

Tali/Legion (Ducts, tested both)
Garrus (First leader)
Samara (Biotic)
Miranda (Second Leader)
Jacob (Escort)

I think the person who escorts the crew always lives if the mission
is succesful.

I used Mordin and Grunt in my team in the end, don't know if that matters.

I went through and my whole team made it out. I have to say though that (if you've fully upgraded it) the "Cain" heavy weapon totally breaks the last boss fight took him out with two shots. It was funny though.

Everyone survived, including my kidnapped crew. All loyal, all upgrades except medical bay. First playthrough.

Door Opener - Legion. As much as Tali was qualified and a genius tech, I picked Legion for two reasons. A) There is no way a single Quarian can outmatch an entire "legion" of Geth operating simultaneously to open the door. B) NO WAY WILL I LOSE TALI!

Strike Leader - Garrus. Chose him because of the elite team he had assembled on Omega and he had specific experience in doing just the kind of operation required. The death of his team was really not his fault. That, and everyone hated Miranda. Gotta have a squad leader you actually want to follow.

Biotic : Samara. Jack was a possible option, but I don't think that she would be able to beat an Asari Justicar. I need Miranda on my team for her squad bonuses as well as heavy warp to dig through Harbinger barrier and armor.

Crew Guard: Thane. Trusted him a lot based on conversations with him, plus I think he could take down just about anybody.

Diversion team: Garrus. Gotta go with Garrus with this one.

On my team: Mordin and Miranda. I was a Bastion Adept. Miranda and I would throw out Heavy Warp at the Harbinger, destroying the barrier and hitting into armor. Mordin would then incinerate. This process usually took less than five seconds unless I missed a shot.

First play through, 2 members died.

Not important who, but they were loyal. I thought they weren't supposed to die if they were loyal. I had read that no one dies as long as you have loyalty, and it doesn't matter who you pick. Read that out of the book, screw you prima games and your useless guides if you were wrong...

Legion and Mordin died due to Samara being team leader.  Use Garrus, Miranda, or Jacob instead.

I survived.
Legion died after going through the ducts. I don't know how other people got him to survive, but I was pretty surprised he got his face sniped. :(
Samara died protecting the strike team. :(
Miranda become my Biotic shield and somehow Garrus died to the swarm. :(
Jacob escorted the crew, they all lived.
I don't remember picking someone to defend the rear as I worked on killing the Human-Reaver, but Doc Morlan died :(
Grunt almost slid off the platform, but I saved him. Not sure if anyone can actually die there.
How did you guys save everyone? I'm sad that I lost Doc Morlan and Garrus.

So, if anyone could correct me the best possible choices are:

Legion/Tali for Vents?

First strike team leader - Garrus?

Biotic barrier - Samara/Jack?

Crew Guard - Thane/Jacob/Mordin?

Diversion team - Garrus?

That sound about right, I want to be able to get everyone alive by the end.

I have bolded and underlined who I picked. I kept Miranda and Mordin in my own squad. Everyone lived, including crew. I can't remember the difficulty, but I think it was veteran.

All squad and crew survived, scariest moment is definitely whether or not the Tech can close the door in time - really nail biting stuff.

(All important quests/upgrades completed before IFF quickly followed by Legion before kidnap attack, Tech-Legion, Team leader-Garrus, Biotic-Jack, Team leader-Miranda, Escort-Jacob)

Well, I ended up using:

Tech Specialist: Legion
Fireteam Leader: Samara (Apparently, despite Miranda agreeing with me... not the best choice since it got Legion killed.)

Biotic Assist: Samara (She did MUCH better in this role.)
Diversion team lead: Garrus
Escort for the crew: Tali (I wanted to keep her safe on the ship since I was worried she was under the weather due to face touching in Shep's cabin.)

Final squad: Garrus and Jack. Sadly, these two didn't really talk a lot. Garrus tried to convince me to keep the reaper data to use, but that's about it. Well, that and Jack yelling at me to hit the tubes.

Losses: Legion
EVERYONE else was saved. The entire crew.

There are a few other choices that work, I'll update those that I had and you don't list here:

Vents - Legion, Tali

First strike team leader - Garrus, Miranda

Biotic barrier - Samara, Jack

Crew Guard - Thane, Jacob, Mordin, Zaeed

Diversion team - Garrus

I'm sure there are other choices as well that could be gleaned from what people have said in this thread.

How? I had 10 loyal squadmates, but Legion died, and only three crewmembers remained to be rescued.

The crew member thing definitely relates to how many missions you did before you rescued them. I should've just reloaded after I did the Reaper IFF thing and everyone vanished. Only Doctor Chakwas survived the first time. :( God, that was sad. Damn my completionist tendencies!

The squad... I'm not sure, but I do feel pretty convinced that it has to do with upgrades and loyalty--and possibly even how often you took them during missions, but I sure couldn't prove that, it's just a wild guess. I kept an eye out during missions and got all the upgrades I could find, then researched them back on the Normandy. I varied my cast of characters during the N7 missions and everything else. Everyone survived. My configuration was...

Techie in the Shaft: Tali--I almost picked Legion, but we had a history and I trusted her brain.
Strike Leader: Garrus--Same as above. History and trust, and I wanted to give him a shot at healing those old wounds that came with getting his old squad killed. I felt he'd reward my faith in him.
Biotic Shield: Samara--Nobody else had her kind of experience and wisdom.
Guard: Grunt--I just asked myself, "Hmm, who can I trust to be a one-man army and barrel through the enemy as he distracts them from defenseless civilians?" Yeah, that was totally Grunt.
Diversion Team: Jacob--He was once Alliance, but a bit of a maverick, so I figured he could go a bit wild without losing control.

And I took Mordin and Thane with me at the end because I worried that they were the weak links; Thane because a dying man may not think it matters whether he survives, and Mordin because I didn't know his age and he was a bit more used to the lab than the field. It felt awesome to keep my whole team alive because I knew how difficult it was; I agonized a little over those choices but it turned out well... yet it was also heartbreaking to hear Doctor Chakwas talk about how it felt  to watch them die, and to see all those emtpy seats afterwards.
