I believe the instructions are on a card that comes with the game…
I’m not sure about 360 in particular, but for PC you go to dragonage.bioware.com/redeem
Your 360 should be linked to the email you use for your EA account (used to log in to above site).
I believe there is an in-game activation that you will need to use the same email address you did for the site to redeem the code. <-- This is very important
