I had my entire team survive. I'm glad i saved before going on the suicide mission. i lost Zaeed my first try, but everyone made it on my second.
Anyone have their whole team survive?

I reloaded the final boss and took the Salarian along, everyone survived.
So, if you have a single weak link, try putting them on your squad.
If half your team gets wiped... You did something very, very wrong (or right, depending on your feelings towards your crew)

I think who lives or dies is pulled from a number of things. Here's my theory:
1) loyalty obviously matters for everyone's survival.
2) Who you choose for each task in the Collector base matters. Some choices work on their own, some choices only work in tandem with your other choice. For example, choosing any tech person will work if, say, Miranda leads the 2nd team but only Tali or Legion will work if Garrus or Jacob leads the 2nd team (this isn't a researched fact and the individuals mentioned might not be the right ones for each task in my theory, just a general guess to indicate my thinking and causality). Also, certain choices for leader might get anyone you send through the vent killed.
3) I think the time you take personally for each section might have an effect on who lives and who dies. IE, if you take too long to reach the end of the swarm section whomever is holding up the biotic shield won't be able to hold the shield and someone will get snatched away as you clear the area or get killed on the other side of the door because you took too long to get to and open it. IE, take too long killing the final boss and one of your active squad members might not have enough time to make it to the exit before the Collector base is nuked.

Only lost Jack, since I didn't get her to loyal again after the row with Miranda.. couldn't care less for her though. Never saw her as part of the crew, was actually seeking the 'space this ****' option, will let you guys know if I find that button.
Kelly is a special case.. if you linger too long after her abduction she get's liquefied.
Haven't tested the tubing part.. I send Tali in, but looking back Legion might have been a better choice, and a safer one too.

When I first played through as a Paragon Soldier I chose all the possible ways to prepare and save others, and tried to steer my crew down the right path. What I think people are missing -although I did see a few people say it before- is that the way you handle the loyalty missions might also have a large impact on whether or not they will survive or which positions that can fit in.
I had everyone loyal to me, upgraded everything a on my first go I lost Mordin in the vents with a head shot. I stopped him from killing his assistant and upgraded him fully, so i restarted and put legion in, he made it through no problem.
On my successful time I went:
Legion - vent
Garrus - fire squad leader
Samara - bubble
Garrus - second team again
Mordin - escort
Miranda and Tali - final team

I told them to hold the position and all of them survived. I had to try 2 times to get it right. I think it's just a matter of making good judgment calls. Who is a good leader, who is a good tech specialist, etc. But the MOST IMPORTANT THING IS THAT YOU MUST SEND MORDIN BACK TO THE SHIP.
Otherwise, he dies for sure. So my choices were...
Tali-Vents
Jacob-Group Leader
Samara-Bubble
Mordin-Escort
Jacob-Group Leader Again
I took Miranda and Garrus with me, though I don't think that makes a difference.

Tali picklock, Miranda lead team 2, Samara for biotic shield, and have Miranda lead again for distraction. And of course, they must all be loyal just to be safe. Send Grunt back with the crew survivors if you go thru the Omega mass relay immediately.

Just finished the game for the first time after 42 hours. Everybody survived *beams*
I did all the loyal missions and got most of the upgrades.
I did it this way:
Tech: Tali
Team leader 1: Miranda
Escort: Jacob
Team leader 2: Garrus
Biotic shield: Samara
Team for final battle: Mordin and Garrus
I made the stupid choice not to destroy the base and got everybody mad at me. So I reloaded the last savegame and did it again. The only thing I changed was Legion instead of Mordin for the last battle and that got Mordin killed :0( So I did it once again....
Seems the only way to keep Mordin alive is to either make him the escort or have him at your side in battle.
I did all the loyal missions and got most of the upgrades.

Guys, stop teasing me!
I chose to wait for the price of this game to drop because I wasn't sure whether I'd like the game or not. The promo vids almost stopped me from buying it. A friend then told me it's not a total shooter as I had initially feared and made me but it. I have no regrets.
I'm used to posting long after everyone else has. It's mostly tv shows, though.

I got all my characters to survive and on impossible.

It would have been more dramatic if some of your squadmates had to die, yes. The way it was on Virmire. Would be more realistic, too. Fat chance everybody survives a crazy suicide mission like that...but it makes you proud to have saved them all. I hope this will be rewarded in ME3.

thats the hope.... that saving them makes a difference in ME3. Thane is perhaps the only squadmate that could legitimately have died without affecting the next game, since he's due to die soon anyway... an ultimate sacrifice to ensure the survival of all the remaining squad members...
except for my first playthrough, where i made the mistake of having Mordin go through the vents, and Garrus got wiped as leader of that 1st squad. And Samara biting it with her as the leader of the 2nd squad... i've not failed to save everyone every time since..
to save ALL the squadmembers ... the following KEY characters must be loyal.. you'll also need the three ship upgrades - armour, shields, and weapons.
Garrus, Miranda or Jacob... choose one or two at least... (on my last playthrough i completely neglected Jacob... didn't use him for any tasks in the Suicide mission, he just helped hold-the-line and he survived that). the person/s you choose must lead the squads. and the first one WILL still die, if the tech-expert is not a loyal Tali or Legion.
Jack or Samara/Morinth to hold the biotic field. choose one, but be warned, if jack is not loyal and is part of hold-the-line, she is likely to die. If you keep her in the final boss fight, and she's not loyal, she'll die. un-loyal Samara/Morinth is likely to survive hold-the-line, but will die in the final battle.
Tali or Legion (and if Legion... its loyalty mission can be only one of the roughly 3 missions you have time for before your Crew gets liquified by your delaying.) use them as the tech-expert.
Escort duty for surviving crew: i'd recommend NOT sending Grunt or Zaeed to escort the crew... send a loyal Mordin, since he has the highest chance of death in hold-the-line.
Hold the Line: grunt and zaeed are built for exactly this type of fighting. Their presence up the chance of survival for your squishier squadies in their fire-fight. Jacob, Garrus, Samara these are also strong soldiers for this position.
Final Battle: this is almost exclusively a matter of personal taste. I prefer to have Miranda (loyal of course) due to her convo with TIM. Tali isn't the best choice, but it ensures a loyal Tali will survive...
Mordin and Zaeed are great choices for this battle due to their incendiary powers being strong against armour. But again, Zaeed should hold the line IMHO. I usually roll with Samara and Miranda, due to personal favourites... though i have used most of the "softer" squadies instead of Samara. (never jacob tho... just don't like him i suppose... he's too boring to get to be with me when i take out the Human Reaper)

I had all the Normandy crew and all my squadmembers alive on my first try...
I didn't anything of special.. just the most "natural" decisions I could take as I play as a Paragon.
Honestly? My reaction was "WTH?! Where is supposed to be the difficulty of this suicide mission?!"

my 1st playthru, i picked grunt as squad leader. after tali got shot in the face, i decided that jacob would actually be useful for once and he got to be squad leader the next time around.
i also tried to pick grunt as tech specialist, but the game wouldn't let me.

I had Garrus of all people die on me, when I stupidly trusted Miranda that she was a sufficiently powerful biotic to do the shield against the seeker swarms.
I declared that to be discontinuity and did a whole another playthrough of ME1 to get a romance-free Shepard to ME2 for Tali.
Never lost a teammate since and that's what really happened.

lol if you're not able to get your entire squad through alive I would revisit your general gaming skills, as Bioware made the suicide mission about as much suicide as choosing between strawberry jam or gasoline as fuel for your car.

I had everybody survive my 2nd playthrough... When I do first playthroughs of games like these I tend to just accept whatever consequences arise from my choices. Unfortunately that resulted in Wrex dying in ME1 and Mordin & Grunt dying on ME2- my paragon's failure
. Because of this I usually go for saving as many people as possible on my 2nd playthrough...which doesn't really seem to make sense because my 2nd character is a renegade and tends to not give a ****...
