I did it a few times and I felt SM was a joke. So no, I won't get my whole team alive in my next playthrough. I'll only get 6 of them alive.

To the person posting above:

Yes, that is true. The game pretty much tells you who to pick for what, not hard at all.

In
fact my friends and I agree that it actually takes much more careful
thought and planning to kill your whole team then to get them all out
alive. In fact I can now memorize all the people I used and in what
order off the top of my head.

If you don't upgrade Normandy at approach to Collector base you already lose 3 squad members.
And much depends if squad members are loyal. So if you don't do loyalty missions good chunk of teammates are gone too. So no planning needed to kill off everyone. Just lazyness.

I had Garrus of all people die on me, when I stupidly trusted Miranda that she was a sufficiently powerful biotic to do the shield against the seeker swarms.

I declared that to be discontinuity and did a whole another playthrough of ME1 to get a romance-free Shepard to ME2 for Tali. :P Never lost a teammate since and that's what really happened. ;)

After reading dossiers at Shadow Broker's base it became pretty clear who is capable of what.
Garrus is excellent leader (only Shepard is better), Samara and Jack are really powerful biotics (only with them as shield one of your two teammates doesn't die) etc

On my second playthrough i'm gonna send Legion into vents and Jack as shield just to see how pissed off she would be

Not doing loyalty missions doesn't mean people will die, it will only mean people will die if you are pick them for doing things or leaving all the squishies holding the line at the end on their own. Technically to get everyone out alive with full squad you only need 7 loyal squadmates. (though obviously 3 of them need to be from a select bunch).

Put it this way, I could even get Shepard out alive with no one loyal and still have a few of squadmates alive even with no ship upgrades and crew all alive too.

Armor - Jack

Shields - Kasumi

Cannon - Thane

Vents - Tali

Swarm - Legion (Samara as biotic, Miranda as 2nd squadmate)

2nd TL - Jacob

Escort - Mordin (crew saved)

Final Battle - Samara and Miranda

HtL Survivors - Garrus, Grunt and Zaeed, everyone else listed above dead apart from crew

Can even swap out one of the HtL Survivors with anyone out of the 'dead' squadmates and all the HtL will still survive even if it was one of the squishies. Because 2 of the HtL listed above will basically ensure the safety of someone else in such a playthrough.

Vents - Why do people think skill at going through the vents is the deciding factor here? You're flat out told that the main problem is someone with technical skills to open the door after the vents.

Fire team Leader - Why the hell would anyone pick Zaeed? Did anyone listen to his stories? EVERYONE DIES but him.

Hold the Line - Mordin prefers to get the job done and go home. Bad for holding the line. Escort good choice, ensures survival.

To get rid of him?

But of course. No I was refering to people who think Zaeed would be the correct choice for a fire team leader.

A better question would have been whose team had casualties. Flawed design if you ask me.

@Bamboozalist and yet by sending Mordin as Escort same people could lose someone else because they still had 2 of the heavy hitters in the final battle, so talking about Mordin on the HtL to some extent is pointless because you could say the same about Tali, Jack or Kasumi seeing as they have same score as Mordin on door defence and are next in line to be squished.

Those would be people who don't read char backgrounds and Shadow Broker dossiers :)
